    Supporting documents that must accompany your application are: 1) a copy of the most recent fiscal year's audited/reviewed Financial Statement and 2) a copy of the proposed project budget. Please provide all available details on income and disbursements. If you hold investments or reserves, please indicate any restrictions that may apply to the use of each fund.

    This section should be attached as a separate document, to allow you to save and edit your documents in advance. Please provide a detailed description of the project being undertaken and how it meets the criteria for the specific grant you are requesting.  Please note whether you see this as a one-time project or an ongoing request. * Please note: grants are for one time only, even if the project is ongoing. Please provide a brief description of your Group/Ministry.  Include such things as the number of people you serve; activities and outreach stories; current staffing, etc. (as applicable).
