I hereby acknowledge by my signature on this agreement that the above referenced dog shall be trained, boarded or otherwise handled and cared for by Annie Wildmoser. I hereby release and agree to hold harmless Annie Wildmoser, any affiliates or property owners from any and all liability, claims, suits, actions, loss, injury or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my dog(s) may sustain or which may be caused in any way by my dog(s). I specifically, without limitation, agree to fully indemnify Annie Wildmoser, any affiliates or property owners for any and all such liability, claims, suits, actions, losses, injury or damage.
I understand that I am solely financially responsible for any damage or harm caused by my dog during boarding, board and train, socializing, playgroups, outings or walks.
I acknowledge that I knowingly and without mental reservation hereby release and hold harmless Annie Wildmoser, any affiliates, property owners and all others who come in contact with my dog from any and all liability, either direct or consequential, arising out of any accidents, injuries, disease, death, escape, theft, injury to persons, injury to other animals, damage to property, or any unavoidable causes by my dog while in the care of Annie Wildmoser. Annie Wildmoser will exercise due and reasonable care while my dog is in her care/custody.
As owner of the dog, I hereby authorize Annie Wildmoser to obtain any medical or surgical treatment for my dog required while in her care/custody at my expense. Including but not limited to: fecal test, nasal/throat culture, flea treatment etc. My regular vet is the one stated on the Intake Form. If veterinary treatment is necessary during my dog’s stay with Annie, she may use my regular vet or a vet known and trusted by her, with every effort made to contact me and my regular vet with any pertinent medical history.
If my dog is diagnosed with any condition which may be contagious, Annie may board my dog at the vet’s office in isolation if warranted at my expense. Annie will also make every effort to reach me as soon as possible to communicate any incident or medical problem that may occur.
I will be solely responsible for any and all costs incurred for treatment of my dog. I also agree to hold the vet, vet hospital, and/or Annie Wildmoser harmless.
I guarantee payment for boarding or the agreed board and train of my dog plus any agreed extras at the time Annie Wildmoser receives my dog. Annie will require a non-refundable deposit of 50% at time of reservation. The remaining balance of 50% is due at the start of the program. Any extras incurred during the stay must be paid when dog is returned to me. If for any reason the bill is not paid when presented, I will be responsible for all costs that Annie incurs in her endeavor to collect payment in addition to the amount of the bill.
If my dog is not called for within 15 days of planned return date, my dog becomes the property of Annie Wildmoser. Reasonable attempts will be made by Annie to collect outstanding monies owed and to return the dog to me or other responsible party of my choosing. Final notice will be by certified/registered mail to the owner at the address given on the Intake Form. No further notice shall be required.
I represent that I am the legal owner of the dog or the legal owner’s agent. I also represent that to the best of my knowledge the dog has not been exposed to distemper, enteritis (diarrhea), Para influenza (kennel cough), parvo virus, canine flu or any other communicable diseases within the past 30 days prior to the stay and has a current (within 90 days of the stay) clear fecal test.
When dogs play together sometimes unintentional injuries result from the play such as bites, scratches, etc. I understand that, while being in the care of Annie Wildmoser, my dog may spend “playtime” with other dogs, and I hereby release Annie Wildmoser from liability for any injuries that may occur.
I understand that while my dog is in Annie's care it will be traveling with her by car and may be traveling in and around the west coast. This travel is a part of my dog's training and boarding.
I have read and agree to the terms as stated in this contract.
