We, as a participant, player and parents of a participant or player competing in the Evanston Youth Baseball League activities, while participating during activities including any pre-game or post-game activities for the Evanston Youth Baseball League that we are agreeable to the following:

We acknowledge that we are aware that there are risks of exposure directly or indirectly arising out of, contributed to, by or resulting :

• An outbreak of any and all communicable disease, including by, limited to, the virus “sever acute respiratory syndrome corona virus 2 (SARS-CoV-2)”, WHICH IS RESPONSIBLE FOR Corona Virus Disease (COVID-19) and/or any mutation or variation thereof;

In consideration of having the opportunity to participate as a team member or competitor for the Evanston Youth Baseball League and in acknowledging that we are aware of and willing to assume the risks associated with this activity, we hereby voluntarily agree to waive, hold harmless and indemnify the Evanston Youth Baseball League, located in Evanston, Wyoming and its trustees, agents, volunteers and employees from any and all claims, demands, damages and causes of action of any nature whatsoever arising out of ordinary negligence which we, our heirs, our assigns or successors may have against them for, on account of, or by reason of our participation in the above activities. We indicate our agreement to this hold harmless elective noted below.
