By providing my credit card information, I acknowledge and agree to a 24 Hour Change or Cancel Fee of $30. This fee will be automatically charged in the event of a no-show for this or any future appointments, or if I fail to provide at least 24 hours notice for a change or cancellation. I also understand and agree not to have any makeup, lotion, or oils on my face during the appointment. In the case of a lash service, I confirm that I will not have any false lashes or lash extensions on my eyes that were not applied by Sunless Beauty. Please be aware that there may be an additional charge for the removal of these items, or if I decline the removal fee, I will need to reschedule and pay a $30 rescheduling fee.*

EYELASH & BROW LIFT, TINT, AND EXTENSION CONSENT AGREEMENT

1. Consent to Procedure:

I, the undersigned, voluntarily agree to have an eyelash lift (perm), eyelash tint, and/or eyelash extensions applied to my natural eyelashes and/or retouched.
By signing this agreement, I consent to the procedure performed by my technician.
I acknowledge that I have been informed of the nature of the procedure, including potential risks and aftercare requirements.

2. Acknowledgment of Risks:

I understand that there are inherent risks associated with these procedures, including but not limited to:

Eye irritation, pain, itching, or discomfort.
In rare cases, eye infection or temporary blurriness.
I agree to contact my technician and consult a physician at my own expense should I experience any adverse reactions.
I acknowledge that although my technician uses proper techniques and professional-grade tools and products, I may experience sensitivity or require follow-up care from a physician.

3. Post-Treatment Care Instructions:
I agree to adhere to the following post-treatment care instructions to maintain the best results:

Avoid water contact with the eye area for at least 24 hours post-application.
Refrain from using oil-based sunscreens, moisturizers, or cleansers on the lashes for 24 hours.
I understand that failure to follow these instructions may result in reduced longevity or effectiveness of the treatment.

4. Returns and Cancellation Policy:

All sales are final; no refunds will be issued for services or products.
In cases of COVID-related cancellations by either party, only an online or store credit will be issued.
Opened or used products are non-refundable.
Unopened products may be returned for store credit only, subject to a 20% restocking fee.
No credits or returns will be accepted after 14 days from the date of purchase.

5. Acknowledgment and Waiver:

I confirm that I am at least 18 years of age, or I have obtained parental consent for this service.
This agreement remains in effect for this and all future procedures performed by my technician.

I have read and understand this legally binding consent agreement in English.
I release my technician from all liability associated with this procedure, acknowledging that it is performed with due care and professional expertise.
I understand that results may vary and that the longevity of the eyelash lift, tint, or extensions depends on various factors.
