The purpose of this agreement is to confirm the services, pricing, and entirety of the day to be rendered for the wedding party. This is to be filled out prior to the consultation and we will go over this in detail during our appointment. If you do not have all the information, that is ok! Now you know what information you need so you can start gathering any missing information needed. This agreement needs to be complete no later than 14 days prior to your Wedding Day.

Please be aware that this contract requires a credit card to reserve all of your appointments. The complete cost of all bridal party services will be figured upon the information you provide on this Contract. The total amount due is to be paid in full no later than 48 hours prior to your wedding.

Gratuity is greatly appreciated, however it is not calculated here on this form, please consider this when making your payment.

1st payment of 50% of the total amount that we calculate on this form during your consultation appointment, be sure to include any additional fees that are applicable. (50$ on location fee, gas fees, travel fees, etc) is due no later than 14 days prior to date of service. (Refer to my website for additional fee information and include in your total amount.)

ADITIONAL SERVICES ARE RENDERED

In the event of additional services, You will receive an invoice for the remaining balance. You will also be notified via text message or email and we can arrange payments via Venmo/cashapp/ etc or I will send you an invoice.

PLEASE NOTE

**In the event that additional, applicable fees are accidentally not included when you are calculating your total amount due. They will be added on either on your last payment or after your wedding day. You will be notified of any charges that will be made to your card so you have the opportunity to ask any questions. However, if I do not receive a response after notification, your card will be charged the appropriate amount.

**In the unfortunate event (this has only happened one time in more than 10 years) that I do not hear back from you, no payment arrangements are made, and there is an unpaid balance for anursing additional services rendered OR any applicable fees that were missed or somehow overlooked, you are still responsible for payment. If my attempts to contact you, and/or make payment arrangements are unsuccessful. There will be an additional $75.00 fee that will be charged to your card, in addition to the remaining balance.

No charges will be made to your credit card without notifying you. By signing this you understand I will charge the card on file after 48 hours of your Wedding Day, whether or not I hear back from you upon notification, if no other payment arrangements were previously made. Each guest may pay individually. They must be paid in full on the day of service. Please note below how you will be paying. All Credit Card information will be used for this event only and the printed version of this form is shredded, as I take all necessary precautions to protect any/all personal information given to me.
Payment Options

Please take payment from selected credit card information I gave above on the date of ___________________________.I'll be making the payment using cash app, square, or facebook pay.I'll be making full payment in cash.Each person will be paying seperately.I will use the Make a Payment section at the end of this contract to make my payments. I understand I can come back to this form at anytime to make further payments.
Cancellation Policy
Any changes or cancellations must be made no later than one month (30 days) prior to wedding date. Cancelation policy is as follows;
*In the event that an appointment is canceled from 14 days to 30 days prior to your appointment, for ANY REASON-you will be held responsible for half (50%) of the total cost of the service(s) that we agreed upon and calculated above. If I do not receive payment 24 hours before your wedding day, your credit card will be charged.
*In the event that an appointment is canceled from 1 day to 14 days prior to your werring day, you will be held responsible for 100% of the cost of the service(s) and your credit card will be charged. Appointment services and times cannot be changed on the day of the scheduled appointments. Doing so will result in the loss of the appointment and a full charge for the service(s) scheduled. It is important that all appointment services and times be closely adhered to, as originally scheduled, so that we may provide the optimal service that you deserve on your special day!

I apologize for any inconvenience, but I must adegree to this policy out of respect for other styliss, MUAs, and other clients.
