Complete this record after you have observed the Probationary TAF deliver, assess or facilitate the Award(s). There is no limit to the number of records that can be completed as each will form the basis for the applicant's eventual endorsement application.

Probationary TAFs must have at least one Co-Delivery Record for each award they are seeking endorsement in.

A copy of this form will be automatically emailed to the Probationary TAF and the Endorsed TAF for record keeping purposes.
