Instructions for Endorsed TAF:
Complete this record after you have observed the Probationary TAF deliver, assess or facilitate the Award(s). There is no limit to the number of records that can be completed as each will form the basis for the applicant's eventual endorsement application.
We recomend completing this form in the presence of the probationary TAF as there is a section both of you need to complete.
Probationary TAFs must co-deliver or co-assess at least one course for the award they are seeking endorsement. For endorsements that cover multiple awards, co-delivery or co-assessing may need to include each award. eg. IRB Endorsements must include Crew & Driver.
A copy of this form will be automatically emailed to the Probationary TAF and the Endorsed TAF for record keeping purposes.
