GENERAL RELEASE *

I agree to pay a NON-REFUNDABLE fee in full prior to training sessions .I understand Monthly Zoom Memberships will be on AUTO-PAY until cancelled. Auto-pay occurs on the same date of every month starting on the date signed up. (For ex. If you sign up on the 5th, auto-pay occurs on the 5th of every month) Monthly Zoom Memberships can be cancelled via email notice (info@bodybylulu.com) at anytime before your due date of the month. (For ex. You must cancel on any day before the 5th for the following month) Cancellation after the due date will be fully charged.I understand there are no credits, make up classes, or paused memberships for missed sessions. Sessions are held on MWF via Zoom and MWF at 9:00 AM at Flowstate Studios located at 4422 SW 73 Ave Miami, FL 33155.I understand there are no refunds/credits due to COVID/illness/injuries.I am aware the schedule is subject to change at the trainer's discretion and there are no classes on holidays.I consent to the use of my image in photographs or videos for marketing purposes.I am voluntarily participating in an exercise program by Luanna Singh dba bodybylulu LLC.I understand that all exercise carries a risk of injury and assume all risks.I expressly agree to release and discharge Luanna Singh dba bodybylulu LLC from any and all claims and/or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action due to all risks that might arise from any negligence on the part of myself or others.This is a general release of all liability allegedly attributed to any and all negligent acts of omissions in bodybylulu personal training.I acknowledge that I have carefully read this "general release" and fully understand that it is a release of liability.
