  • If you have multiple programs under your agency group (e.g. pantry, meal site, etc), you must submit an individual form for each program. You will have an option to submit an additional form upon submission. If your agency/program number is not working, please contact partnerships@secondharvestsw.org.

    For pantries: if you are receiving funds directly from Second Harvest to support your school pantry program, please choose School Pantry Program. Otherwise, please choose Food Pantry.

    You will need to enter your program number correctly to submit this form. 

    • Enter your 5 digit program number and press the "Auto Complete Program Fields" button to autofill in your program information.
  • PROGRAM REPORTING

    Please provide number of visits or number of unduplicated households, or both. If you collected more information, we welcome you to share this as well. (Unduplicated = unique households, e.g. if a household visits your pantry 4 times a month, their visits would count for 4 times in Number of Households Served, but only 1 time for Unduplicated Households Served)
