This form must be submitted within 7 days of move in. Please note, only one Tenant per unit need fill out this form. It is the Tenant's responsibility to document any damage upon move in. All maintenance requests should be submitted via your tenant portal. If you have an emergent maintenance situation please call 719-581-RENT and select the option for maintenance.
