Exotic Pet Boarding Form

With exotic pet boarding you are required to bring your pet's enclosure, bedding, etc. We require this so that your pet will have their familiar housing while away from home. The cost of boarding also covers daily medication administration if this is something that your pet requires (all medications need to be in the original labeled containers with your pet's name, strength and frequency of administration).

In order to help prevent items from being misplaced in the hospital we ask that you label all of your pet's belongings with their name prior to their arrival. Animal Care will not be held responsible for items misplaced or damaged.

Payment is due in full for all services when your pet is released on departure day. During peak holidays, a deposit may be required to reserve your pet's boarding reservation.

I acknowledge that Animal Care of Greenville is a "flea-free" facility and, in order to maintain this, each boarding patient will be assessed for fleas upon arrival and given the appropriate medication to kill any live fleas found at the owner's expense.
I acknowledge that boarding departure hours are Monday through Friday: 7:30am - 11:30am and 2:00pm - 5:30pm. If pets are not checked out prior to final pick-up time on their departure date, additional nightly fees will be added to the account.
I acknowledge that charges are per night (not per day). There is one charge per pet, per day, even if the pets are housed in a single enclosure. There is a charge for the first night of arrival, regardless of the time of arrival.
I acknowledge that proof of vaccinations that meet the requirements of all Animal Care of Greenville boarders are required. If proof is not provided, doctors of ACG will administer necessary vaccines, including required physical exam, at the owner's expense.
I acknowledge that Animal Care of Greenville will attempt to contact me in the event that unexpected medical attention is deemed necessary by the medical staff.

Non-emergent Medical Care Policy

In the event that a non-emergent medical problem arises while your pet is staying with us, Animal Care of Greenville will attempt to contact you, at the emergency contact numbers that were given to us, prior to administering medical treatment. If we have not heard from you within 24 hours, Animal Care of Greenville will proceed with treatment deemed medically necessary. *

Ferrets: Must be current on Rabies

All vaccines must have been administered by a licensed veterinarian. Pets may have the exams and vaccinations completed at Animal Care of Greenville, or the client must present written proof of current vaccinations/tests for each pet prior to boarding & grooming services. Although obviously sick or unvaccinated pets are not permitted to receive boarding and grooming services, the client acknowledges that they are placing their pets into an area in which communicable diseases may pass between seemingly healthly pets. If following a provided service date, the pet shows signs of illness, the client may not hold Animal Care of Greenville liable for any inadvertant exposure the pet may have received, and that if the client requests an exam, treatment, or medications for the pet that it will be provided by Animal Care of Greenville at full cost to the client.

ACG Vaccine Policy *

If I have not provided proof of current vaccinations or my pet is not current on the vaccines listed above, I understand they will be given and I will be responsible for these additional charges when I pick up.

Flea-Free Policy *

I understand that if my pet is found to have fleas on the day they are admitted to the hospital, that they will be treated for fleas at my expense. This enables us to keep our hospital flea-free.

Photo Release Form

We would love to feature your pet on our social media sites, etc. - but we need your permission! Please read below and let us know if we have permission to take pictures and videos to capture your pet's experience with our team. Don't worry, we will always get their good side!

I am the owner / agent of the owner of the animal identified above, and do hereby consent and agree that Animal Care of Greenville, its employees, or agents have the right to take photographs, videotape, or digital recordings of my pet and to use these in any and all media, now or hereafter known, and exclusively for the purpose of putting pictures of our patients on our website, social media, and within our newsletter. I further consent that my pet's name and identity may be revealed therein or by descriptive text or commentary. This consent remains in full force and effect unless and until I provide a written revocation of the consent. I do hereby release to Animal Care of Greenville, its agents, and employees all rights to exhibit this work in print and electronic form publicly or privately. I waive any rights, claims, or interest I may have to control the use of my pet’s identity or likeness in whatever media used. I understand that there will be no financial or other remuneration for recording my pet, either for initial or subsequent transmission or playback. I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement. *

INFORMED CONSENT: I hereby consent and authorize Animal Care of Greenville to care for my pet in what way they deem appropriate. I understand that by placing my pet in their care that I agree to allow them to use their best judgment in working with my pet. I understand that my pet will not be directly monitored outside of normal business hours and I assume responsibility for injury, loss, or destruction by my pet regardless of the circumstance.
