A successful adoption depends on selecting the right dog for your household and having a good understanding of the dog's needs and care requirements. Please help us with this process by answering the following questions fully and completely.

This is an extensive application and your efforts are truly appreciated. Your application will be valid for 6 months and can be used for any of our rescue animals—only one application necessary.

Submitting this application does not commit you to adopting, nor does it guarantee adoption approval. It is simply a tool we use to help both you and us identify a good potential match for your family and lifestyle.

Most Adoption fee's range from $350-$450. The adoption fee is not a sale price, but a request to defer or reimburse rescuer for the medical care and expenses of the animal while he or she waits for a new home, as well as food and transportation costs.

These fees help to provide for the other animals in the care of the rescue who may have medical bills that are much higher than any possible adoption sale price. The money also acts as a donation to help support our organization as a whole, allowing us to continue our efforts to rescue and rehome animals in need across the country.

In the event that the dog you have applied for will need to be transported to you, a non-refundable deposit of $200-$250 is due before transportation has begun. This fee applies to direct adoptions as well foster to adopt agreements.
I understand that completing this application does not guarantee I will be allowed to adopt a dog, and that the application is a tool to help Saint Mutts Rescue determine if this dog and my family are a good fit for each other. I acknowledge that all information provided on this form is true and correct. I understand that any misrepresentations may result in my not being approved and/or removal of the adopted dog from my possession. I also certify that I have never been convicted of animal cruelty or neglect.

I agree to allow a representative from Saint Mutts Rescue to contact my vet for a reference and also agree to allowing a home visit prior to adopting this dog. If I am approved to adopt a dog, I agree to pay an adoption donation and hand sign an adoption contract at the time of adoption.

I understand that all dogs adopted from Saint Mutts Rescue will be spayed/neutered at the appropriate age.
