Volunteer Eligibility Requirements

In order to participate in any Eight Waves Programs, volunteers must be at least in Middle School. Families with younger children can volunteer together. Volunteers must commit to spending a minimum of four hours a month with the mentee. All volunteers agree to attend mentor trainings, are willing to communicate regularly with the Program Coordinator, and submit monthly meeting and activity information. Volunteers must have access to reliable transportation. It is required that all volunteers have never been accused, arrested, charged, or convicted of child abuse or molestation. All volunteers are required to pay $25 to cover the fees for our required background check and volunteer t-shirt.
Eight Waves Volunteer Code of Conduct: Eight Waves is a youth-serving, community-based organization dedicated to providing waves of opportunities for vulnerable at-risk children, schools, and families in under-resourced communities. Participation in the organization’s programs is subject to the observance of the organization’s rules and procedures.The activities outlined below are strictly prohibited.Any participant or staff member who refuses to sign will not be allowed to participate or who violates this Code is subject to discipline, up to and including removal from the program.Abusive language towards a staff member, volunteer or another participant.Possession or use of alcoholic beverages or illegal drugs on property or reporting to the program while under the influence of drugs or alcohol.Bringing and dispensing of any medications to children. Bringing dangerous or unauthorized materials such as explosives, firearms, weapons or other similar items.Discourtesy or rudeness to a fellow participant, staff member or volunteer.Verbal, physical or visual harassment of another participant, staff member or volunteer.Actual or threatened violence toward any individual or group.Conduct endangering the life, safety, health or well-being of others.Failure to follow any agency policy or procedure.Bullying or taking unfair advantage of any participant.Failing to cooperate with an adult supervisor/leader/mentor.I have read and I understand Eight Waves’ Code of Conduct. I agree to abide by the rules described above and understand that I may be removed as a participant if I violate any of these rules.

Volunteer Applicant's Criminal History Background Consent
It is the policy of 8 Waves Corporation to conduct criminal history background checks for individuals who are eighteen years of age and older, and participate in volunteer activities. If criminal history is revealed that was omitted from your application, you will be disqualified. YOU MUST DISCLOSE ALL CRIMINAL HISTORY. Applicants must disclose this information even if told differently by a lawyer, judge, or other law enforcement individual. Prior criminal records may or may not result in disqualification for volunteering; however, failure to disclose this information will disqualify you for volunteering. Information that must be disclosed includes any offense that occurred whether as a juvenile or adult; therefore all adult and juvenile offenses must be listed.

I agree and consent that 8 Waves Corporation (using an external background check provider) can conduct a criminal history background screening on me. I agree to waive any and all claims against 8 Waves Corporation, Florida, its agents, board members and employees for any decision made with respect to my volunteer status as a result of the criminal background screening.

I hereby authorize, without reservation, any law enforcement agency, administrator, state, municipal or federal agency, motor vehicle agency, institution, school or university (public or private), information service bureau, employer, or insurance company to furnish any and all background information requested.

You will not be placed with or allowed to serve in any volunteer positions until you have been approved based upon the results of your background checks.

VIDEO/PHOTOGRAPH RELEASE FORM

I hereby grant Eight Waves the irrevocable right and permission to use photographs and/or video recordings of me on Eight Waves and other websites and in publications, promotional flyers, educational materials, derivative works, or for any other similar purpose without compensation to me.

I understand and agree that such photographs and/or video recordings of me may be placed on the Internet. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video recordings of me. I waive the right to approve the final product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of the Eight Waves.

I hereby release, acquit and forever discharge Eight Waves, its current and former trustees, agents, officers and employees of the above-named entities from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said photographs and/or video recordings, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.

I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name or, if I am less than eighteen years old, that my parent or guardian has signed this release form below. This release is binding on me and my heirs, assigns and personal representatives.

RELEASE AND WAIVER OF LIABILITY

This Release and Waiver of Liability (the “release”) executed on _____________, 20__, by ________________________________ (“Volunteer”) releases Eight Waves Corp., a nonprofit corporation organized and existing under the laws of the State of Florida, and each of its directors, officers, employees, and agents.

The Volunteer desires to provide volunteer services for Eight Waves and engage in activities related to serving as a volunteer with Eight Waves.
Volunteer understands that the scope of Volunteer’s relationship with Eight Waves is limited to a volunteer position and that no compensation is expected in return for services provided by Volunteer; that Eight Waves will not provide any benefits traditionally associated with employment to Volunteer; and that Volunteer is responsible for his/her own insurance coverage in the event of personal injury or illness as a result of Volunteer’s services to Eight Waves.
1. Waiver and Release: I, the Volunteer, release and forever discharge and hold harmless Nonprofit and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the services I provide to Eight Waves. I understand and acknowledge that this Release discharges Eight Waves from any liability or claim that I may have against Eight Waves with respect to bodily injury, personal injury, illness, death, or property damage that may result from the services I provide to Nonprofit or occurring while I am providing volunteer services.

2. Insurance: Further I understand that Eight Waves does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health or disability benefits or insurance of any nature in the event of my injury, illness, death or damage to my property. I expressly waive any such claim for compensation or liability on the part of Nonprofit beyond what may be offered freely by Eight Waves in the event of such injury or medical expenses incurred by me.

3. Medical Treatment: I hereby Release and forever discharge Eight Waves from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my tenure as a volunteer with Eight Waves.

4. Photographic Release: I grant and convey to Eight Waves all right, title, and interests in any and all photographs, images, video, or audio recordings of me or my likeness or voice made by Eight Waves in connection with my providing volunteer services to Eight Waves.

5. Other: As a volunteer, I expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Florida that this Release shall be governed by and interpreted in accordance with the laws of the State of Florida. In the event the laws of the State of Florida conflict with each other regarding a specific relevant issues, this Release shall be governed by and interpreted in accordance with the laws of Florida. I agree that in the event that any clause or provision of this Release is deemed invalid, the enforceability of the remaining provisions of this Release shall not be affected. By signing below, I express my understanding and intent to enter into this Release and Waiver of Liability willingly and voluntarily.
