- As you prepare files for uploading, please name all files using this naming format:
  - [Student First Name] [Student Last Name] [Student ID #] [File Type]
  - I.e. John Smith 1234567 Graduation Plan
  - Sign and complete where necessary
  - ***Do not combine multiple documents into one file. Only the grad plan should be in the grad plan file, only the transcript should be in the transcript file, etc.

You will see a checkmark next to Bacterial Meningitis Vaccination item on your new student checklist from your Admissions tile in myLoneStar indicating whether you have already completed this.

Official shot records and medical exemptions are submitted from your Admissions tile in myLoneStar by clicking on the Bacterial Meningitis Vaccination checklist item. The Dual Credit office can not submit these for you.

This is NOT required if the student is ONLY taking online courses.

Compliance Options
