    (Only for students who consider LSC-Montgomery, LSC-Conroe Center, or LSC Creekside their home campus.)
  • Hello Lone Star College-Montgomery Maverick!  We are ready to serve you. 

     

    Required Registration Items:

    • Updated Homeschool Graduation Plan
    • Updated Homeschool Transcript
    • Completed Exceptional Admissions Course Approval Form (Submit a separate form for each term. I.e. Summer 26, Fall 26, Spring 27, Summer 27.)

  • Home Campus Status:

    Students should register for classes through their home campus dual credit office.

    The home campus should be set to:

    • Where the student attends most classes
    • Where the student would like to process paperwork through
    • Where the student will receive advising
    • Where they would like to graduate from

    This form is for students who claim LSC-Montgomery, LSC-Conroe Center, & LSC Creekside as their home campus.  If none of those are your home campus, please do not complete this form and instead reach out to the dual credit office from your home campus for further assistance.

    This Partnership Agreement Form must be completed initially when first beginning the dual credit program.

  • Have you completed all items on the Homeschool Student Checklist?*
    • If you answered, "No" or "I'm not sure," please visit the dual credit website and prepare all forms from the Homeschool Packet as separate PDF's before proceeding.

    Note:

    • Attach all files as PDFs. There are many free Scanning Apps to help you create PDFs.  
    • As you prepare files for uploading, please name all files using this naming format:
      • [Student First Name] [Student Last Name] [Student ID #] [File Type]
      • I.e. John Smith 1234567 Graduation Plan 
      • Sign and complete where necessary
      • ***Do not combine multiple documents into one file.  Only the grad plan should be in the grad plan file, only the transcript should be in the transcript file, etc.  

    Registration will be processed in the order it is correctly received. 

    Not following these instructions will delay your registration and you will be required to resubmit your paperwork!

  • Please check that your Graduation Plan is:

    • Named [Student First Name, Last Name, Student ID, Grad Plan)
    • Typed
    • Signed by the homeschool administrator
    • Includes any Lone Star Classes that have been or will be taken for every single grade level (including past grade levels).
      • The classes on the student's grad plan and transcript will be identical for grade levels he/she has already completed.***Note that the graduation plan is due every time you register.

    • Named [Student First Name, Last Name, Student ID, Grad Plan)
    • Typed
    • Signed by the homeschool administrator
    • Includes any classes that have been taken, including Lone Star Classes.

    ***An updated homeschool transcript is due every single time you register.

    ***If your student is an upcoming 9th grader, you must still complete this, though it will be sparse and likely only include personal contanct information.

    • Signed by the homeschool administrator AND student
    • Includes:
      • class #
      • Subject
      • Catalog #
      • Section #
      • Semester/Year that you are registering for (Have a separate form for each semester)

    Dual Credit students are NOT permitted to take classes that require a concurrent class. This is noted in the notes of a class when searching for classes and can also be identified if the class section number includes a C in it.

    We encourage you to pick classes that have several seats left as it takes time to process your paperwork.

  • Upload Bacterial Meningitis Records (If Needed)

    You will see a checkmark next to Bacterial Meningitis Vaccination item on your new student checklist from your Admissions tile in myLoneStar indicating whether you have already completed this.  

    Official shot records and medical exemptions are submitted from your Admissions tile in myLoneStar by clicking on the Bacterial Meningitis Vaccination checklist item.  The Dual Credit office can not submit these for you.

    This is NOT required if the student is ONLY taking online courses.

     

    Compliance Options

    1) Submit Official Shot Record.

    2) Submit proof of Medical Exemption.

    3) Submit proof of Conscientious Objection.

    Note: It may take up to 2-3 business days for any option to be processed.

  • Degree Plan Declaration/Changes

    As a dual credit student, you are required to file a degree plan with Lone Star College after completing 15 credits or 2 semesters, whichever comes first. Even if you plan to transfer to a 4-year university after you graduate high school and prior to completing a degree at Lone Star, you MUST declare a degree plan with Lone Star College.  

    If you need to make a change to your degree plan or if you are currently Non-Degree seeking and need to claim a degree because you have completed 15 credits or 2 semesters, please visit this link to do so.  

     

  • CERTIFICATION

    By providing my signature below, I certify that all the information I have submitted is true and correct and that I am the student or the student's authorized parent or guardian. I authorize Lone Star College to register me (the student) on my behalf based on the courses provided on my registration form, or the alternate courses provided in this online form. 

    **Students will receive a confirmation email when registration is completed
