I hereby give my child permission to participate in the Thrive Peake Cheerleading program. I have read the Season 6 Parent Informational Packet and I fully understand my obligation, as well as my child's. I also understand that my child must be covered by personal insurance. I give my permission for my child to recieve medical attention in the event I cannot be present or reached for any reason. I understand there is a $125 registration fee upon submission that secures my childs spot.

Authorization and Release
* I authorize Thrive Cheerleading and its representatives to consent to medical treatment for my child when I cannot be reached for consent. No prior determination of life-threatening emergency or danger of serious or permanent injury resulting from treatment need be made under this authorization. Exceptions to this authorization are as follows:

* I fully understand that cheerleading and/or tumbling can be a dangerous activity. I am fully aware that any activity involving motion, height, or athletic activities creates the possibility of serious injury, and I further agree to hold Thrive Cheerleading and its staff and officers harmless for any injury or resulting expense(s). I release and discharge all rights and claims against Thrive Cheerleading and its parties. Thrive Cheerleading strives to provide the maximum in safety procedures and guidelines, and cannot assume responsibility for any accidents, injury or illness that may occur.

* I authorize Thrive Cheerleading to use photographs, video and/or other likenesses of my child for use in its promotional materials or sales and waive any rights of compensation or ownership thereto.

In consideration ofblanks* , my minor child/ward ("My Child"), being allowed to participate in any way in Thrive Cheerleading program, related events, competitions and activities, the undersigned acknowledges, appreciates and agrees that:

1.    Cheerleading is a vigorous sporting activity involving height and rotation in a unique environment, and, as such pose a significant risk of injury, Cheerleading and related activities always involve certain risks, including but not limited to death, serious neck and spinal injuries, and serious injury to bones, joints and muscles. The risk of such harm may be limited by safety equipment and coaches, but will never be eliminated,
2.    For myself, spouse, and My Child, I knowingly and freely assume all such risks, both known and unknown, and even if arising from negligence of Thrive Cheerleading or others, and assume full responsibility for My Child's participation;
3.    I willingly agree to comply with Thrive Cheerleading standard and customary terms and conditions for participation in Thrive Cheerleading programs. If I observe any unusual and significant concern in My Child's readiness for participation and /or in the Thrive Cheerleading program itself, I will remove My Child from participation and bring such issue to the attention of the nearest Thrive Cheerleading personnel immediately;
4.    I myself, my spouse, My Child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE THRIVE CHEERLEADING, ITS EMPLOYEES, TEACHERS, COACHES, AGENTS, SUBCONTRACTORS, OTHER PROGRAM PARTICIPANTS, SPONSORING AGENCIES, SPONSORS, ADVERTISERS, AND IF APPLICABLE, OWNERS AND LESSORS OR PREMISES USED TO CONDUCT THRIVE CHEERLEADING PROGRAMS ("Releases"), with respect to any and all injury, disability, death, or loss or damage to personal property incident to My Child's involvement or participation in Thrive Cheerleading programs, whether arising from the negligence of the Releases or otherwise, to the fullest extent permitted by law; and
5.    I, for myself, my spouse, My Child and on behalf of my/our heirs, assigns, personal representatives and next of kin HEREBY INDEMNIFY AND HOLD HARMLESS AND CONVENANT NOT TO SUE all of the above Releases, from any and all liabilities incident to participation and involvement in Thrive Cheerleading programs, even if arising from the negligence of Releases, to the fullest extent permitted by law.

This Agreement is made and entered this day blanks* (Date) of

   Name* (Responsible Party). The Responsible party, as parent/legal guardian of   Name*     (Athlete) hereby agrees as follows:
1.     I understand the commitment, standards, and financial obligation involved with joining a team and becoming a member of Thrive Cheerleading.
2.     I understand that competitive cheerleading is a commitment and it is my intention to remain at Thrive Cheerleading for the entire season. I further understand that if I chose to remove my athlete for any reason or if he/she is removed from the program for any reason, all fees including, but not limited to, choreography fees, coaches’ fees, competition fees, uniform, practice wear, and tuition are NON- REFUNDABLE.
3.     I agree to pay all fees associated with being a member of Thrive Cheeerleading from the time of joining until my athlete is officially removed from the program. A 30-day written notice is required to be officially removed from the program. A withdrawal form can be obtained from the coach. Your account will continue to be billed until this notice is received.
4.     I understand it is my responsibility to keep up with my account. Statements of the athletes account can be obtained by request. I understand that failure to receive statements does not mean I am not responsible for the charges to my account.
5.     I understand that my balance must be paid in full by the 5^th of each month. I further understand that if my balance is not paid by the 5^thof that month, my account will automatically be assessed a late fee of $15 per week.
6.     I understand that if my account becomes delinquent, my athlete may be asked to sit out of practices and competitions until the account is brought current. I also understand that if my account becomes more than two months’ delinquent my child will be dismissed from the squad and legal action may be taken in which the responsible party will be required to pay all legal fees. Furthermore, my child will not be allowed to return the following season on a payment plan. Full payment will have to be made up front.
7.     I understand that due to the nature of many fundraising policies, any credit or overpayment on my account from fundraising proceeds will be forfeited if my athlete withdrawals, or is removed, from the program for any reason.
8.     I agree that my athlete and I will adhere to the policies and standards established by Thrive Cheerleading. I realize that failure to do so may result in my access to the Thrive Cheerleading facility being restricted or my athlete being withdrawn from the program.

TERMS AND CONDITIONS

Monthly payments are a compilation of known costs of participation in The Thrive Cheerleading Program including, but not limited to, registration, cheer tuition, competition fees, choreography/music fee, and Uniform package and any other applicable fees.

Once your payment is recieved, the Handbook is signed and Birth Certificate is recieved, your child will be officially registered as Thrive Peake Cheerleader.
