By asigning This wavier Form, I Acknowledge the following
I confirm that I have 3 days following my install to inform the stylist at Its A Hair Thang 865 of any concerns or changes I would like to my install.
I confirm that I will follow the regimen and the suggested follow-ups of the salon in maintaining and treating my hair. 2
I understand that the length of time I can wear these depends greatly on the current condition of my hair as well as how well I follow the suggested maintenance routine.
I am aware that some protective styles can be damaging to my hair and Ashlee M Jones is not responsible for any damages
I hereby release any and all persons representing this salon from all claims,demands, damages, actions and cause of action arising out of the performance of my service
I am allowing the salon to apply necessary products as part of the service in my hair treatment.
I consent the salon to take photographs of the provided service.
I consent the salon in terms of sharing the photograph to social media for marketing campaigns or testimonials.
I agree to pay the booking fee which is 50% of my services and I understand that this fee is non-refundable.
I understand that if I NO CALL NO SHOW with NO Communication at all for my hair appointment my card will be charged %50 of the remaining balance
I understand that I can only reschedule my appointment one time after that I know I will Forfeit my booking fee and will have to rebook with a new booking fee to move forward
I have read this whole document along with the terms and conditions on Ashlee's booking site by signing this I accept the terms indicated above as well the terms and conditions on Ashlee M Jones’s booking site.
I understand that if I or Ashlee M Jones has to reschedule our hair appointment due to illness or family emergency I know and agree that my Booking fee of 50% of my services is Non-refundable under No circumstances and my appointment will be rescheduled only
