AMPED Birthday Party Policies
***Please read carefully.

AMPED Birthday Parties are available to guest of ALL ages. Each birthday party includes the following:

1) use of the facility for 2.5 hours including tables/chairs.

2) a designated AMPED party host who guides participants through a 1 hour dance class and 30 minute arts and craft activity for up to 10 guest and one birthday dancer.

3) use of the sound system and LED lights/strobe lights.

4) set-up time 1 hour before party start time and clean up 30 minutes after party end time.

Dance Styles: You can choose from Hip-Hop, Jazz, Modern or Sensual Dance (18+ with heels recommended).

Additional Guest: Spectators are allowed to attend the party, however they will not be able to particpant in the guided craft unless they pay the additonal crafter fee. ***Due to building restrictions no more than 45 guest are permitted in the studio at any time.
AMPED Birthday Party Cost
Cost: All birthday parties must paid in advanced at least two weeks before the party. Amped To Dance reserves the right to cancel your party if payment is not received by the studio within this timeframe.

The total cost is $450 per party. The additional crafter fee is $15 per person. Upon receiving your completed registration an invoice will be sent for payment.

***Please note there is a small fee for all credit/debit card purchases. To avoid the service charge you may pay for upcoming lessons in cash after your first session.

Acknowledgments: By signing this form you acknowledge that Amped To Dance will not be held liable for and will be held harmless from any and all liabilities, losses, injuries, damages, or expenses related to the Amped Birthday Party at the Studio. You have read this release and waiver of liability and indemnity clause, and agree that no oral representations, statements, or inducements apart from this foregoing agreement have been made.

Publicity Release: You hereby grants permission to Studio, to use photos and videos of Student for advertising and publicity purposes, inclusive of print advertising, educational video, television, website, video taping or film broadcasting in connection with promotional campaigns.
