Quest Theatre Survey

We are looking for your help to shape the future of Quest Theatre with your participation in a short survey.

This survey is being conducted by Mary Blais, an external consultant, in order to receive your feedback about your experience with the programs and activities of Quest Theatre.

The responses will be used to inform decisions the staff and board will make for the organization going forward, in relation to its programs and activities.

The survey is intended to be anonymous. You will not be asked for your name or contact information. Please do not include any personally identifiable information (e.g., name) in any of the open text fields.

The survey will take approximately 5 to 20 minutes, which will depend on how detailed a response you provide.

Only the first four questions are mandatory. The rest are optional. We invite you to skip over any other questions you are not comfortable responding to.

The survey will be available until 11:59pm on March 13, 2021.
