Please complete the following form to apply for a library account.

FOR SPANISH FORK RESIDENTS: This application is for a temporary account, which will expire 3 months after activation. If you would like to continue your temporary account, you will need to come to the library and present a valid form of ID. If you already have a library account, do not complete this application.

FOR NON RESIDENTS: Non residents are required to pay to use the Spanish Fork Library. Non residents may select between a 12-month ($100) or a 6-month membership ($50).
