Why must I complete an order authorization form? - To help prevent our business from fraudulent orders we ensure we are dealing with the correct customer.This form allows us to insure the customer who placed the order is the authorized card user.

If you do not submit your form within 5 business days we will CANCEL & REFUND the order.

IF YOU ENTER ANY INCORRECT INFORMATION (scribbled signature, wrong order info, wrong ID, etc) IT MAY CAUSE A DELAY since we will reach out to you.

Just to be clear, we completely understand that other companies may not request information from you. But due to previous issues we've had with fraudulent customers, this is a protective measure that can efficiently protect our business from those issues occurring again.

What to do in different scenarios

1. The name & address on your ID is the same as your billing name & address. You only need to upload your ID.

2. The name on your ID matches the billing name but the address does not. Upload the ID that confirms the name & ADD a piece of mail with the same billing address entered at checkout which confirms this address is an authorized billing address.

3. You have NO mail or NO ID. Email customer service at support@pinkyvirginextensions.com & let us know so we can figure out a solution
