Jerzy's Never Forgotten Rescue understands that many families have two working adults and we don't expect anyone to be a super hero and quit a day job just to be able to take home a puppy/dog. Every puppy/dog family has different needs, and this information form helps us identify who needs what information and what homes might make the best fit. If you're a veteran dog owner, we don't want to waste your time regaling things you already know; by the same token, if this is your first puppy/dog, we want to make sure you leave prepared for what life with a puppy or dog may bring. Please remember this is a lifelong commitment and should not be a spur of the moment decision! JERZYS NEVER FORGOTTEN RESCUE reserves the right to deny any application.

Ownership Commitment
When adopting you will get the chance to asses the dog. You must take full responsibility of the dog and the dogs actions once adopted! Jerzy's Never Fogotten Rescue gives out information to the best of our knowledge and can not be held responsible or liable once the dog is adopted! Do you understand and do you agree to these terms? What to expect
Completing this application is a step in the process to acquiring a puppy from JERZYS NEVER FORGOTEN RESCUE but it does not guarantee you get a puppy or dog from Jerzy's Never Forgoten Rescue. JNFR reserves the right to refuse any placement at any time.

APPROVAL AND DEPOSIT: We will review applications within 2 business days of receipt. Applications are processed in the order they are received. If your home is a good fit, we notify you. We only accept deposits on dogs or puppies that are available after the application is accepted. Please make this $25.00 deposit within 48 hours of the approval e-mail, this goes towards the adoption fee. If you make a deposit, you are committing to take the puppy or dog, if you decide not to take the puppy or dog the deposit is non-refundable and will go towards the care of other animals at JNFR

YOUR PICK OF PUPPY/DOG: Puppies/Dogs will be assigned in deposit order by the first or second preference of the depositor as indicated on the application. We ask that you make a choice within 24 hours of notification since there may be other families waiting for a chance to choose. Should you elect to not take a puppy, the deposit is forfeited and the deposit is applied to the costs of other animals at the rescue.

GOING HOME: We cannot hold dogs or puppies for extended times; it is critical for their social development that the dogs or puppies are in homes within a reasonable timeframe. If you have a big vacation planned or have events that would conflict with a young puppy, please know puppies cannot be boarded in a kennel until they have received their full immunizations and their rabies vaccination which cannot be done until 3 months of age.

If Jerzy's NF Rescue has a specific breed that you are intrested in or adopting please be knowledgable about that breed! IE French Bulldogs can not swim,hearding breeds need to have a job and will not be happy sitting around all day.!

Any dog or puppy from our rescue always has a home here, and will never experience the fear and burden of life at a shelter so please be sure to contact us with any problems.
