    By submitting, you're letting us know you're ready to move forward with fostering through LifeLine Animal Project. This questionnaire helps our team learn more about you so we can match you with the most suitable foster pet.

    We are only able to accept foster parents who live in the state of Georgia. If you are a full time resident of another state, we encourage you to foster with your local shelter or rescue organization.

    • Foster Commitment and Preferences

      To better match foster parents with animals in need, it would help us to know your preferences for any future foster cases.
    • Heartworm
      Heartworm disease is a non-contagious condition that is spread through mosquitoes. Worms form in the heart and constrict the heart's ability to properly pump blood to the rest of the body. Heartworm treatment is covered by LifeLine! The treatment requires administering oral medication for 30 days, followed by 2 injections, and 30 more days of low activity (crate rest and short walks). The full treatment is a minimum 2-month commitment.

    • Medical Needs
      By selecting this option, you’re offering to care for animals that may be recovering from surgery, illness, or injury. This can include administering medications, monitoring healing, managing mobility issues, or providing extra rest and care. These fosters may require more time, attention, and communication with the rescue or vet, but your support is critical in helping them heal and prepare for adoption.

    • Behavioral Needs
      By selecting this option, you’re opening your home to animals who may need extra patience, structure, and training. These pets might be shy, anxious, undersocialized, or have specific behavioral challenges. Fosters in this category often work on building trust, providing consistency, and helping the animal learn that the world is a safe place. Your role is key in preparing them for a successful adoption.

    • Special Needs
      By selecting this option, you may be caring for animals with chronic or long-term conditions such as FIV, FeLV, diabetes, or mobility issues. These pets may require ongoing medical monitoring, a stable environment, and sometimes limited contact with other animals. While they may need a bit more care, many live happy, loving lives—and your support gives them the second chance they deserve.

    • Fospice
      Foster + hospice = Fospice. Fospice care focuses on easing the pain and symptoms of a dog's terminal illness. We prioritize comfort and quality of life by reducing pain and suffering. This is best achieved in a foster home. We guide you through every step of the way as this can be an emotional experience.

      To help us better match animals with foster pet parents, please tell us about your home environment. Please note that these questions will not determine your foster inquiry.

    • This agreement is made and entered into effect on the date stated above by and between LifeLine Animal Project (hereinafter referred to as "LifeLine" and “the organization”), inclusive of its operations at Fulton County Animal Services (FCAS), DeKalb County Animal Services (DCAS), and the Community Animal Center (CAC), and the Fosterer named above (hereinafter referred to as "Fosterer"). By signing below, the Fosterer agrees to, voluntarily and upon consent, temporarily care for a homeless animal(s) on behalf of LifeLine and to abide by the following regulations:

      General Rules and Guidelines

      Fosterer understands that the foster pet is only temporarily in their care and remains the property of LifeLine Animal Project, specifically the location from which the animal originated (FCAS, DCAS, or CAC). Any and all adoptions of the foster pet will be handled through LifeLine.

      While the Fosterer is encouraged to assist in the placement of the animal, the final adoption decision rests solely with LifeLine. LifeLine or its designee reserves the right to remove the foster pet from the Fosterer at any time, with or without prior notice.

      LifeLine shelters are licensed by the Georgia Department of Agriculture (GDA). Because foster homes are an extension of the shelter, foster homes must pass GDA inspection. The Fosterer agrees to an initial and periodic inspection by LifeLine and/or GDA.

      The Fosterer shall provide the foster pet with humane care and maintain him/her in accordance with all current state, county, and municipal laws and ordinances while the animal resides with the Fosterer. The Fosterer agrees to provide the foster pet entrusted to their care with adequate food, water, shelter, affection, socialization, and exercise as they would their own pets.

      The Fosterer shall provide the foster pet with a fenced yard or other humane means of exercise.

      The foster pet shall not be kept constantly tethered or outdoors. Foster dogs shall be leashed at all times when outside an unfenced area. Foster dogs are not allowed to visit dog parks or other off-leash areas. Foster cats shall be housed exclusively indoors.

      The Fosterer understands that the organization will not reimburse for any property damage incurred due to the foster pet's presence in the home.

      The foster pet shall reside at the Fosterer's address. If the Fosterer cannot keep the foster pet, the Fosterer shall notify LifeLine immediately. The foster pet shall not be sold, abandoned, relinquished to a shelter or to another rescue group other than the foster pet’s LifeLine shelter of origin. Fosterer will not relinquish custody of the foster pet to anyone other than LifeLine. No person under the age of 18 shall attend to or care for a foster pet without direct parental supervision.

      Fosterer agrees to be contacted by LifeLine representatives via email and phone to communicate relevant information that can include, but is not limited to, foster pet medical needs, adoption events, and updated program protocols.

      Fosterer understands that LifeLine has disclosed all known information regarding the foster pet and understands that the information provided to Fosterer about the foster pet may have been received by LifeLine from third parties, that this information cannot be relied upon, and that LifeLine does not warrant the accuracy or correctness of such information.

      Fosterer agrees that if they are fostering an unaltered pet from LifeLine, they will not have any other unaltered pet within their residence, unless they have received prior approval from LifeLine's foster staff.

       

      Marketing and Adoption

      Fosterer agrees to provide, or coordinate assistance to provide, marketing materials about their foster.

      In the event that the Fosterer wishes to permanently adopt the foster pet, an Adoption Inquiry must be filled out and approved by an Adoption Counselor, and, if approved, an Adoption Contract must be signed and submitted along with the appropriate adoption fees.

      In an effort to find foster pets the best home quickly, LifeLine may put Fosterer in direct contact with potential adopters. Fosterer agrees that their email be connected to LifeLine’s Adoption Inquiry software and to receive emails about adopters interested specifically in Fosterer’s foster pet. Fosterer is required to reach out to interested adopters within a reasonable period of time and schedule meet and greets with interested parties. Fosterers are expected to follow an Adopter's Welcome protocol during this process. LifeLine will not share any other personal information of Fosterer with the adopter, and LifeLine will not share Fosterer’s personal information with parties outside of LifeLine Animal Project's overall operations.

       

      Medical

      Fosterer understands the foster pet may be incubating infectious diseases unbeknownst to the LifeLine veterinarians. Fosterer also understands that LifeLine will not provide medical care or treatment to Fosterer's personal pets should they contract any disease from the LifeLine foster pet.

      Fosterer agrees to follow all veterinary treatment protocols and dispense medications as prescribed by LifeLine’s veterinarians. The Fosterer will make no decisions or actions regarding the foster pet's medical care (including euthanasia when an animal is put to sleep to end their life) without prior approval from LifeLine. All well and sick visits will be scheduled at the LifeLine foster pet’s shelter of origin. In the case of an emergency, Fosterer will make every effort to contact the Foster Coordinator or the Shelter before considering an emergency visit. LifeLine will not reimburse for unapproved emergency vet visits.

      LifeLine will continue to provide preventative care (including vaccines, dewormer, flea and heartworm preventative) and medications for most illnesses as prescribed by a LifeLine- approved veterinarian. Fosterer may be responsible for costs related to any care obtained without prior LifeLine approval, as well as for specialized diagnostics or treatments not recommended by LifeLine veterinarians.

       

      Behavior

      The Fosterer understands that LifeLine and its representatives have limited information regarding the temperament and habits of the foster pet. The Fosterer will take all reasonable precautions (through the use of crates, leashes, supervision, and common sense) to ensure that the foster pet does not run loose, become a nuisance, destroy property, injure or become injured by another animal, or injure or become injured by any person or persons.

      Fosterer understands that animals can be unpredictable, and that LifeLine cannot anticipate or ensure the conduct of animals fostered from LifeLine. Fosterer acknowledges that LifeLine has not made through its agents, volunteers, or employees, any representations or warranties regarding the condition, health, temperament, or conduct of the animal.

      Fosterer understands and acknowledges any known behaviors of the foster pet and confirms they have been counseled on this information prior to signing this agreement. Fosterer agrees to maintain behavior training protocols supplied by LifeLine for the foster pet should it be deemed necessary by LifeLine.

       

      Liability & Delinquency

      The Fosterer agrees to release and hold harmless LifeLine Animal Project (including FCAS, DCAS, CAC, and its representatives) from any claims, damages, or liabilities that may arise from fostering. This release applies to any events or incidents arising from or related to the care or conduct of the foster pet during the foster period.

      In the event that LifeLine is required to pursue legal action to enforce the terms of this agreement, the Fosterer agrees to be responsible for any reasonable legal fees and court costs incurred by LifeLine as part of that enforcement.

      In the event that the Fosterer becomes non-responsive to repeated communication attempts by LifeLine for a period exceeding 21 days, or fails to disclose the whereabouts or condition of the foster pet upon request, such actions will be considered a material breach of this agreement. LifeLine reserves the right to take any and all legal action to recover the animal and terminate the Fosterer's participation in the program. The Fosterer may also be reported to local animal control authorities, and any future involvement with LifeLine may be prohibited.
