Thank you for your interest in the FCOM 2021 Poster Submission for Residents, Fellows, and Students. The 2021 Florida Combined Otolaryngology Meeting (FCOM) will be held at the Ritz-Carlton, Naples, FL, as well as virtually, November 5-7, 2021.
Please complete all of the following application information which is required for CME approval, including a Conflict of Interest/Disclosure form for EACH presenter/author. Download the form here, and complete it ahead of time, so that you can upload the form(s) into this application via file upload below. Be sure to also save the completed form. The conflict of interest form needs to be completed separately for each presenter/co-author, meaning you may be submitting multiple attachments in the file upload below. However, only the presenter needs to submit an application.
Applicants will submit an abstract for their posters via the prompt below. If accepted, presenters will be given 8-minutes to present and discuss their posters. The top three posters will receive prizes.
If you are presenting multiple posters, you must submit a new form for each poster.
Please note: You must be a student, resident, or fellow at the time of the poster presentation in November.

CME activities are conducted in the public interest. Therefore, it is important to assure the public that education received by physicians is conducted with the highest integrity, scientific rigor, objectivity, and the absence of commercial bias. All faculty of FMA-sponsored activities are expected to disclose financial relationships with any commercial interest that produces, markets, re-sells, or distributes health care goods or services consumed by or used on patients, that are related to the content of the educational activity in which they are involved.
The intent of this form is to inform faculty members of content validation requirements and to resolve potential conflicts of interest assuring balance, independence, objectivity, and scientific rigor in all CME activities.
All submissions are due no later than July 1, 2021.
