TRIO Programs Application

This assessment contains a number of statements about students needs while enrolled in college.  Please provide your honest opinion so that a SSS staff member can determine the best methods to help meet your needs. All answers provided during this assessment are kept confidential and will only be used to make your time at WVSU successful.

Student Agreement

The purpose of this agreement is to communicate an understanding of your responsibilities while attending West Virginia State University and participating in Student Support Services. We believe students should be aware of their responsibilities before agreeing to participate in the program. Thus, we request you carefully read this agreement before signing.

Certification

I certify that any information that I have provided in this application is true to the best of my knowledge. I understand that Student Support Services staff will use the data provided on this application form to assist in assessing any academic and/or career planning needs. Student Support Services staff may obtain any and all information and documentation necessary for processing and ongoing evaluation of my academic pursuits. This includes, but is not limited to, the following departments: Admissions, and Records, Financial Aid, Collegiate Support Services-Disability, and other departments on the West Virginia State University campus. I also understand that the Student Support Services staff can assist me in achieving my academic goals only if I fulfill my obligations and that failure to meet my responsibilities as required can result in dismissal from the program. Student Support Services or West Virginia State University staff may include my name, statements or photo in publications including brochures, recruiting materials, web sites, press releases, Student Support Services social media site, and other publications which feature the Student Support Services program or highlight participant’s achievements and accomplishments.
