    Distinguished Alumni Award Nomination Form

  • The Distinguished Alumni Award (DAA) is one of the highest honors bestowed upon alumni of Saint Mary-of-the-Woods College. Since 2003, this award has been presented to the most prestigious alumni of SMWC for their professional achievements, contributions to society and support of the College. These individuals and the accomplishment of their life's work serve as a source of pride and inspiration for the students, faculty, staff and alumni of the College.

  • Nominations are due by January 15.

    Incomplete nomination forms will not be accepted for consideration.
  • The criteria for the award include:

    • A nominee must be an alumnae or alumnus of the College; and
    • Must have shown exemplary career accomplishments, professional impact and community involvement; and
    • Must have shown service to the College philanthropically or through their volunteer work; and
    • May be awarded posthumously.

    Nominations will be accepted from alumni and friends of Saint Mary-of-the-Woods College.

  • Please describe how the nominee fulfills the criteria of the Distinguished Alumni Award by answering the following questions in a clear, concise, one-page letter of nomination.

    • Community Service
      • Please list all community organization(s) and, in particular, leadership roles that nominee has been involved in, along with number of years of involvement.
      • What new programming or innovations has nominee been responsible for in the area of community service? 
      • What other awards and/or recognition has nominee received (local, state, national) for community service?
    • Service to Saint Mary-of-the-Woods College
      • Please be specific in listing areas in which the nominee has served Saint Mary-of-the-Woods College. In what ways has the nominee gone 'above and beyond' as an alum in service to SMWC? (i.e. volunteer services, recruiting efforts, mentoring of students, involvement with a particular academic department, etc.)
      • How many years has the nominee been devoting this service to the College?
      • Has the nominee received other formal recognition by the College for service?
    • Professional Achievement
      • Please be specific in listing areas of achievement by nominee: professional positions held and noteworthy accomplishments within that profession.
      • Please list any national, state or local recognition for professional accomplishments of nominee.
      • Does the nominee hold national, state, or local offices on boards or organizations relating to his/her profession? If so, please list.
      • Does the nominee regularly contribute articles for publication, service in a consultative role or hold speaking engagements in connection with his/her professional expertise? If so, please detail as much as possible.
