Your counselor will also work with you during your first two sessions to begin to develop the criteria for discharge once your counseling has been completed. To be considered for discharge, you must meet one of the following conditions: Achieve the individual treatment goals judged to be necessary in order to be discharged from treatment Symptoms and level of functioning have improved to the point that you no longer require counseling sessions to maintained improved level of functioning. Your clinical condition has become such that you require a higher level or intensity of care. You demonstrate a lack of motivation to participate in the agreed upon individual plan. For example, having a poor record of attendance at scheduled sessions, poor record of completion of any assignments, no follow through to community based support groups, or non-compliance with any outside medical recommendations to stay on prescribed
Should you be discharged from services, a list of alternative agencies from which you can seek services will be provided to you. Once you have been discharged from services, a written transition/discharge plan will be completed by your service provider and a copy will be provided to you. If applicable, your caseworker or probation officer will also be provided with a copy of your plan regardless of whether or not you successfully completed treatment. In the event that an unplanned discharge occurs, an independent staff person will contact you to provide necessary notifications, clarify the reasons for the unplanned discharge, determine with you what further services are needed, and offer or refer to needed
If you are discharged due to aggressive or assaultive behavior, a staff person will contact you within 72 hours of discharge to ensure that you have a link to appropriate services.
