For this commercial project, I hereby state and represent that;

• I am experienced in and familiar with services I am performing;
• I hold a current, valid, Driver’s License and Commercial General Liability insurance;
• I fully understand the risks and dangers inherent in work I am completing;
• I am voluntarily participating in services I am completing, knowing the potential conditions and factors associated with preforming these services; and
• I expressly agree to assume the entire risk of any personal injury which I might suffer as a result of my participation in carrying out services
• I agree to the Scope of Work on this Auxe Work Order and associated total job technician payout (including all taxes). If additional expenses are to be used for this project that are purchased out of pocket by the technician, Auxe may reimburse in full upon receiving a copy of the receipt.

BY SIGNING BELOW, I DISCLOSE THAT I HAVE READ, UNDERSTAND, AND AGREE TO THE TERMS AND CONDITIONS STATED HEREIN AND BELOW. THIS IS A RELEASE – PLEASE READ BEFORE SIGNING:

I, the Client, acknowledge that I have received, in perfect condition, all products and services from Auxe, via an independent contractor representing Auxe Inc., as per all arrangements made. I acknowledge these goods were received without any defect and without damage to my home, business, property, or injury to any persons. 

If applicable, I acknowledge that Auxe has adhered to any privacy laws, or other laws, surrounding the installation of CCTV systems in residential homes. The placement and location the cameras were installed was determined by myself, the owner of this location. Auxe will not provide any monitoring services for my cameras view them at any point.

By signing this form, I agree to hold Auxe Inc., its officers, owners, employees, and independent contractors representing Auxe Inc., entirely free from any liability, including financial responsibility for injuries or damage. This acknowledgement of the receipt of goods and services is valid and clear.
