Application Checklist

All Applicants

To be considered for admission to Five Branches University, your application package must include the following items. For more details, refer to the Admissions section of the catalog or website, www.fivebranches.edu

1. Application for Admission (completed in full)
2. Recent passport-style photograph
3. Statement of Purpose
4. Resume
5. Application fee
6. Health Evaluation
7. Hepatitis B Vaccination form
8. Tuberculosis Test form

Supporting Documents - sent separately

1. Two letters of recommendation
2. Official transcripts of all college and/or university coursework that apply toward your degree or for transfer credit purposes. If you have obtained your education outside of the US, please refer to the International Students section.

Transfer Applicants(Transferring from a TCM/acupuncture school)

Applicants who wish to transfer from another TCM college must submit the following materials (in addition to the materials indicated for All Applicants):

1. Cover letter stating your reasons for seeking transfer and the courses you wish to transfer
2. Additional letter of recommendation from a previous TCM/acupuncture instructor
3. Catalog of the TCM college where you studied, stating its approval agencies and accreditation
4. Syllabus outlining the course content for each course you wish to transfer

International Applicants

Students who will require an I-20 to apply for an F-1 international student status through Five Branches University must submit the following documents (in addition to the materials indicated for All Applicants):

1. A photocopy of the first page of your passport (also of dependents, if applicable)
2. Foreign Transcript evaluation by a credential evaluation agency that is a member of the National Association of Credential Evaluation Services, Inc. (NACES) e.g. Educational Records Evaluation Service (ERES).
Applicants with foreign Chinese Medicine education who are interested to sit for the NCCAOM exam, please check the educational requirements with NCCAOM.
3. Test of English as a Foreign Language (TOEFL) or International English Language Testing System (IELTS) scores
4. International Applicant Financial Statement (Form is available online or upon request).

International Applicants (Currently studying in the U.S)

International applicants who are currently on an F-1 student visa and enrolled at another college must submit the following documents (in addition to the materials indicated for All Applicants and International Applicants):

1. A photocopy of your visa, contained in your passport (also of dependents, if applicable)
2. A photocopy of all your previous I-20 Forms (also of dependents, if applicable)
3. International Student Transfer Release form (applicable only after acceptance to Five Branches University)
