  • All Applicants

    To be considered for admission to Five Branches University, your application package must include the following items. For more details, refer to the Admissions section of the catalog or website, www.fivebranches.edu

    These are the initially required documents. Please have the following documents ready when you begin filling out this application form:

    1. Application for Admission (completed in full)
    2. Digital Photograph- passport style
    3. Statement of Purpose
    4. Resume
    5. Non-refundable Application Fee: $100 (Payable via PayPal using Visa, Mastercard, Discover, or American Express only)
    6. Photocopy of Government-issued ID (Driver's Licence or Passport)

    7. Two letters of recommendation 
    8. Official transcripts of all college and/or university coursework that apply toward your degree or for transfer credit purposes

    Supporting Documents - sent separately and directly to the admissions office of your chosen program. Click Here to start your Supporting Documents Form.

    9. Health Evaluation
    10. Hepatitis B Vaccination Proof
    11. Tuberculosis Test form
    12. International Applicant Financial Statement (In the case you are an international applicant)

  • Transfer Applicants(Transferring from a TCM/acupuncture school)

    Applicants who wish to transfer from another TCM college must submit the following materials (in addition to the materials indicated for All Applicants):

    1. Cover letter stating your reasons for seeking transfer and the courses you wish to transfer
    2. Additional letter of recommendation from a previous TCM/acupuncture instructor
    3. Syllabus outlining the course content for each course you wish to transfer 
    4. Catalog of the TCM college where you studied, stating its approval agencies and accreditation upon request from admissions office

  • International Applicants

    International applicants applying for an F-I student visa through Five Branches University must submit the following documents (in addition to the materials indicated for All Applicants):

    1. A photocopy of the first page of your passport (also of dependents, if applicable)
    2. Transcript evaluation by a member of NACES
    3. English proficiency test report with score
    4. International Applicant Financial Statement

  • International Applicants (Currently studying in the U.S)

    International applicants who are currently on an F-1 student visa and enrolled at another college must submit the following
    documents (in addition to the materials indicated for All Applicants and International Applicants):

    1. A photocopy of your visa, contained in your passport (also of dependents, if applicable)
    2. A photocopy of your latest I-20 form
