2021-2022 Parent/Guardian Code of Conduct

CG Athletics is making a strong effort to create a positive and encouraging environment for our athletes. The Parent/Guardian Code of Conduct has been created to ask our parents to be active partners in helping the staff of CG Athletics to succeed in these efforts. We ask all of you to join us in making the 2021-2022 season an amazing experience for everyone at CG Athletics!
I will abide by all gym policies and procedures.

I will support and encourage ALL CG Athletics athletes at practices and competitions.

I will remain in the spectator viewing area of the gym as well as at competition venues. I will not enter any of the training areas without being invited by a staff member of the CG Athletics staff.

Alcohol consumption at competitions or any CG functions MUST be kept to a minimum. Any alcohol related incidents can result in your athletes dismissal from CG Athletics.

2021-2022 PARTICIPATION AGREEMENT AND WAIVER FORM
CG Athletics recognizes that it is our obligation to make our athletes and their parents aware of the risks and hazards associated with the sports of tumbling and cheerleading. The risk of harm may be lessened by all of the equipment, training and precautions, but it is never eliminated. Athletes may suffer injuries, possibly minor, serious, or catastrophic in nature.
While CG Athletics enforces safety rules, it is the parent's responsibility to warn their child about the dangers of and possible injury from these sports. Parents should make their children aware of the possibility of injury and encourage their children to follow all safety rules and coaches' instruction.

Therefore, in consideration of my child's participation in CG Athletics' classes, events, and activities whether conducted on or off the premises of CG Athletics,Iagree to be bound by the following:

LIABILITY WAIVER AND RELEASE

I am fully aware and understand that participation in the programs at CG Athletics involve movement, rotation, and height which carries a risk of injury, including catastrophic injury, paralysis, and even death, as well as other damages or losses associated with participation in tumbling, cheerleading and other physical activities. I understand that I need to provide medical coverage for my child, and I will be responsible for all medical and related bills that may be incurred from said activities. I agree to release CG Athletics along with the coaches and the employees from responsibility or liability for any losses or damages that occur as a result of my child's participation in any CG Athletics' practices, events, competitions and activities or travel for the purpose of participation in any such programs or events. I understand that this waiver extends to injuries incurred by any member of my family.

READINESS TO PARTICIPATE

My child will only participate in CG Athletics' practices, events, competitions and activities for which I believe he/she is physically and psychologically prepared.

MEDICAL ATTENTION

I hereby give my consent to CG Athletics to provide customary medical/athletic training attention, transportation and emergency medical services should CG Athletics' staff deem this to be necessary during the course of my child's participation.

TEAM PAPERWORK AND COMMUNICATION

I have read and signed the CG Athletics Cheer Team Payment Agreement. I understand that I am financially committing my child for the entire 2021-2022 season. I have read and signed the Parent/Guardian Code of Conduct. I will abide by and follow the items listed. I have read and reviewed the Athlete Code of Conduct with my child. I understand all the items listed and will do everything I can to ensure that my child will abide by all of the items listed. Any failure to abide by the Parent/Guardian Code of Conduct and/or the Athlete Code of Conduct may result in the dismissal of my child from the CG Athletics Cheer Teams and I will forfeit all fees. I also understand that the primary form of communication from CG Athletics is E-mail. It is my responsibility to read all communications from CG Athletics and to contact the office if I am not receiving any communications from CG Athletics.

CONSENT TO PAY

I am fully aware of and understand the payment policies and procedures of CG Athletics. I understand that I need to submit a non-refundable annual registration fee to complete registration. I understand that payment is due in full each session prior to the start of my child's 1st scheduled class. I am aware that my child will not be allowed to participate until their account is paid in full by the 15th of each month. I am aware that I will be charged a $20 late fee per month for all payments that are overdue, a $45 fee for all returned checks, and a $30 fee for all credit card payments that are declined. I understand that all fees must be paid within 1 week of notification. If I would like to participate in "Auto' payments, I need to provide a completed credit card billing authorization form. I am aware that it is my responsibility to keep the credit card information up to date. I understand that I need to notify the office 2 weeks prior to my child's last class if I would like to drop from the following session. I agree to pay a $100 drop fee if I do not meet this deadline and assume financial responsibility for my child's class until the office is notified.

MEDIA RELEASE

Without compensation to me or my child listed, I hereby grant CG Athletics and its Affiliates permission to publish and use photographic portraits, pictures or videos of my child for use through reasonable promotion of CG Athletics. I hereby waive any right to inspect or approve the finished media material as long as the material is within reason and is not deemed to be socially inappropriate for use.
AUTHORIZATION TO PARTICIPATE

As a parent or legal guardian, I confirm that the information provided is correct. I hereby verify by my signature below that I fully understand and accept each of the above conditions. I have reviewed and agree to comply with all the policies and procedures of CG Athletics and authorize my child listed to participate in said activities. This agreement shall remain in effect as long as and whenever my child participates in any activity at or with CG Athletics.

2021-2022 Acknowledgement Form

A CG Athletics staff member has communicated and provided the following information to me regarding CG Athletics Competitive Cheerleading as well as gym policies and procedures.

1. Season Schedule and Cancellations:

season schedule is included in team handbook & available online

consistent attendance is required at practice & advance notice of an athlete's absence is requested additional practices may be scheduled as needed weather cancellations will receive notification via mass text

2. Team Tuition Payment Policies and Due Dates:

all payments must be submitted no later than the 7th of each month $20 late fee is applied if payment is not received on time or if paperwork is submitted late athlete is not allowed to participate after the 10th of the month until tuition is paid in full athlete may be replaced for an upcoming competition or exhibition if tuition is not paid in full "Auto' pay is requested, but other payment types are: Visa, MasterCard, Discover, Check & Cash $45 check return fee, $30 card denial fee, $100 Quit Fee A credit card will be kept on file and ran if payment is not received by the 10th of the month

NO fees will be prorated for missed practices & NO refunds of fees Event Fees are non-refundable if your child is unable to attend a competition this is a financial commitment and time commitment to be a part of CG Athletics for the entire 2021-2022

injuries must provide medical documentation so we can follow progress & practice safely. A doctor's note will be required before your child will be allowed to return to participation in practice, competitions or exhibitions.

4. Practice/Competition Etiquette:

athletes are required to wear a designated outfit each practice including bow athletes are required to wear their uniform with designated bow at competition/exhibitions conditioning is assigned to athletes without proper attire or late arrival/missed practice arrive 5 minutes early & be ready to go on time carry shoes into practice/competition/exhibition & change into them once inside the venue hair must be up and secured away from the face for practice & as assigned for competition/exhibition: team make-up is required for all competitions/exhibitions & is to be applied independently NO jewelry on or taped while at practice or in uniform - CG Athletics is not responsible for lost jewelry NO food or drink allowed in the gym, unless approved by Eric or Coaches but should bring a container (with lid) to use water purification system athletes are expected to show team support by staying to watch all teams compete & receive their awards

submit a copy of your athlete(s) official birth certificate and all Team Handbook paperwork support the team hotel rooming block when there is a commitment to use a minimum percentage of rooms spectators should not interact with athletes during practice spectators should remain off the equipment & siblings need to sit with their parents in the viewing area
