By completing and signing this Publicity and Likeness Release (the “Release”), I hereby grant permission, authorize and provide consent, without reservation, to JFNA to take, use and disclose my: (i) name (or any part thereof); city and state of residence; and (ii) photograph, image, digital recordings, videotape, audiotape, film, or any other medium, likeness, voice, and identifying information associated with such recordings and all materials created by or on behalf of JFNA that incorporate any of the foregoing (collectively, the “Image Materials” and together with “Personal Information,” the “Materials”), to describe and to use the same for the following purposes: promotion of good will, public education, and/or commercial purposes, including fundraising, and other related activities of JFNA.
I hereby irrevocably permit, authorize, grant and license to JFNA, its officers, directors, agents, employees, affiliates, independent contractors, licensees and assignees (including photographers), as well as any event sponsors, participating organizations and their respective directors, officers, employees, agents, successors and assigns ("Authorized Parties") the rights to display, publicly perform, exhibit, transmit, broadcast, reproduce, record, photograph, digitize, modify, alter, edit, adapt, create derivative works, exploit, sell, rent, license, otherwise use, and permit others to use any materials in perpetuity throughout the universe in any medium or format whatsoever now existing or hereafter created, including but not limited to, print publications, electronic media, video, promotional materials, press releases, the internet and other digital transmission or delivery methods, without further consent from or royalty, payment, or other compensation to me (the "Rights").
JFNA shall be the exclusive owner of all rights, including copyright, in the Image Materials. I hereby irrevocably transfer, assign, and otherwise convey to JFNA my entire right, title, and interest, if any, in and to the Image Materials and all copyrights and other intellectual property rights in the Image Materials arising in any jurisdiction throughout the universe in perpetuity, including all registration, renewal, and reversion rights, and the right to sue to enforce such copyrights against infringers. I acknowledge and agree that I have no right to review or approve Image Materials before they are used by JFNA.
I hereby irrevocably waive any claims and rights relating to all liabilities, actions, suits, damages, and expenses (including claims for copyright or trademark infringement, infringement of moral rights, libel, defamation, invasion of any rights of privacy (including intrusion, false light, public disclosure of private facts, and misappropriation of name or likeness) violation of rights of publicity or any other similar claim or cause of action in any jurisdiction throughout the world (“Claims”) arising from or related to any Authorized Parties’ exercise of its rights under this Agreement and forever release and discharge the Authorized Parties from liability under such Claims.
I confirm that I have read this Release and that I am fully familiar with, and understand, its contents. I represent and affirm that I have the authority to grant the Rights outlined in this Release, and that I have not previously sold or assigned those Rights. I certify that I am of full age and competent to contract in my own name and to sign this Release. I understand that, if I am under 18 years of age, this Release must be co-signed by my parent or guardian. I agree that this Release shall be binding on me, my spouse, heirs, executors, administrators, assigns, and legal representatives. I have voluntarily signed this Release, signifying my intent to be legally bound herein, by clicking the designated “I accept” button on the Jotform registration page. I agree that such electronic acceptance is the legal equivalent of my manual signature on this Release.
