This form is intended for students who wish to take classes through CSM directly through CSM's Dual Enrollment program, and should be completed by both the student and parent.

This form is not for registering for the dual enrollment option for eligible classes offered in the high school.

This form is not for applying to the Early College program.

You are able to upload your required admissions documents here.
Students seeking admission into the Dual Enrollment program are required to submit a recent copy of their high school transcript to CSM in order to demonstrate that they meet the program's minimum GPA requirements.

Students who do not meet the program's cumulative 2.75 GPA requirement, but who have at least a 2.50 GPA will need to supplement their transcript submission with SAT/ACT scores for consideration on a case-by-case basis.

Please note that a grade report is not an acceptable substitute for the required transcript.
