    IMPORTANT:

    You may request your documents here. Kindly make sure the student has updated their accounts with the Accounting Department (0968-853-2941 or accounting@lics.edu.ph).

    Depending on the type of documents requested, allow 5 to 14 working days for processing. You may contact the Registrar's Office (0968-853-2942 or registrar@lics.edu.ph) to get updates on the status of your request.

    NOTE: There will be a processing fee depending on the type of document(s) requested.

    You will receive an email or a text message on your mobile number with a reference number once your documents are ready for pick-up.

    Documentary stamps and processing fees will apply for some types of documents. The Registrar's Office will inform you of any applicable fees after your request has been reviewed.

    Some documents require that they be sent directly to the new school. Ensure that the school's email address is indicated clearly in the supporting documents which you will need to upload later.

    Important:

    Form 137 or SF10 - A request letter from the new school is a required attachment.
