Band Absence Request

Before filling out this form, please talk with Mr. McDonald First.

Please fill out this form anytime you have an absence from band, even if it has been excused through the school. Please be sure to fill out the correct date for the absence. This will help us maintain accurate records and in turn, ensure that we enter your grade correctly.

Please note that filling out this form does not automatically excuse an absence. Just as when you miss school, you must provide specific details for us to determine whether or not an absence is excused. Absences will be excused in accordance with WISD policy and the WHS Pure Gold Band handbook.

Excused Absences (no deduction from grade; requires advance communication with Directors):
- Conflicting school UIL activity (excused from rehearsal only)
- Excused absence from school
- Death in the family
- Immediate family wedding

Unexcused Absences (deduction from grade):
- Work/babysitting
- Too much homework, school project, studying
- Transportation problems
- Other appointments, meetings, etc.
