• If you moved into the area less than three years ago you must seek a reference from your previous parish and upload it as part of this application process.

• If you are yet to obtain the reference from your previous parish, please wait until you have acquired it to begin completing this form.

• Once you have printed / received the Certificate of Catholic Practice for all the schools you are applying to, please deliver them - (in one envelope) - through the letterbox of The Priests' House.

• Please do NOT ring the doorbell! We receive hundreds of forms for processing, so you can drop them through the letterbox.

• Once you have submitted your form to The Priests' House there is nothing more for you to do.

1. Catholic Baptismal Certificate(s) of the child(ren) you are applying on behalf of.

2. A Catholic Baptismal Certificate of one parent / carer.

3. A photograph of your family - including the child and all parents / carers - which will be used to help verify your identity.

If you are unable to upload all of these documents you cannot complete this form!
