Affidavit of Eligibility and Waiver of Liability

The parent/guardian waiver must be completed and signed by applicant's parent/guardian for submission to be considered.
I,  {parentName}  , hereby attest and affirm that my child, {studentName}, meets the criteria necessary for entry into the National Energy Foundation rEV Challenge and has entered the National Energy Foundation rEV Challenge with my expressed consent. By signing below, I attest and affirm and state under threat of perjury the following:

4. I am submitting this Affidavit to National Energy Foundation with the understanding that the National Energy Foundation will read and rely upon this Affidavit.
5. I represent that my child or legal ward is eligible to win the National Energy Foundation rEV Challenge.
6. I represent that I have reviewed and satisfied all the eligibility requirements set forth in the rEV Challenge Official Rules, which I acknowledge having read and understood the rules, and any other requirement imposed by law.
7. I hereby represent that I have read, have complied with, and will continue to comply with all the rules, regulations, terms, and conditions set forth in the Official Rules.
8. I have not perpetrated and will not perpetrate any fraud or deception in connection with the contest and have not sought to influence the outcome other than by participating as expressly permitted in the Official Rules.
9. I have been given a full opportunity to review and analyze the rEV Challenge Official Rules, I fully and completely understand all of the terms of this Affidavit and of the rEV Challenge Official Rules and sign this Affidavit voluntarily, freely, and knowingly.

I have been told and understand that if airfare is provided to me that the National Energy Foundation, in its sole discretion, will select the cities of departure and arrival between which I shall travel and it shall be up to me, at my sole cost, to arrange for transportation from my home to the city of departure and then home at the end of the trip.
11. I understand that I am waiving any and all liability of the National Energy Foundation for any known or unknown injury to me or the minor Grand Prize Winner in any way associated with the National Energy Foundation rEV Challenge or any travel paid for by them. This waiver is to be broadly applied and encompasses any type of injury whatsoever in any way related to the rEV Challenge.
