Volunteer Form PVSA 2022-2023

The Bavaria Community and Spouses´ Club is lucky to have you as our volunteer! We could not support our Welfare programs and give back to the Community without you.

As a token of appreciation to our volunteers, the BCSC has been granted authority to award the President's Volunteer Service Awards.

The President's Volunteer Service Awards will be awarded based on age and volunteer hours for the 12 month timeframe matching our BCSC´s Board Year from 1 June 2022 through 31 May 2023.

To be able to award our outstanding volunteers with the President's Volunteer Service Awards we are required to collect the information below to verify your eligibility.

If you are volunteering with more than one organization on Tower Barracks or Rose Barracks you are welcome to submit your volunteer hours for your other organization to combine your hours.

I also volunteered blanks hours with blank and would like these volunteer hours recognized for the total hours volunteered from June 2022 through May 2023. I will upload my DA4713 or DA4162 or a statement from a POC below to verify my volunteer hours with another organization.

Please upload your service record with other organizations (e.g. Private Organizations, Units/SFRGs) below. If you log your hours in VMIS hourly please upload DA4713, if you log your hours per period please upload DA4162. If you are unable to log your hours through VMIS please upload a statement from your other organization verifying your volunteer hours starting 1 June 2022.

I volunteered more than 4000+ hours over my lifetime and am eligible for the Lifetime Achievement Award. Please contact me for more info about the verification process for the Lifetime Achievement Award.
