Document Checklist
1. RCMP Certificate for Waiver and Privacy Act (if applicable) Please ask for list of locations;
2. Any court or police documentation from your offence(s);
3. Any documentation received by immigration authorities;
4. To the best of your ability, a detailed account of the circumstances leading up to and surrounding your offence(s);
5. Copies of all criminal convictions;
6. For drug related offences/convictions, recent drug test results. Please provide list of locations;
7. A copy of your Birth Certificate;
8. All pages of your current passport;
9. A copy of your Driver’s Licence
10. A copy of your Marriage and Divorce Certificate;
11. A copy of your spouse’s Birth Certificate;
12. Copy of your child(ren)’s Birth Certificate (if applicable);
13. Waiver questionnaire;
14. Names and contact numbers for 3 or more individuals who can serve as references and who have personal knowledge of your character and reputation in the community. Preferably, we’d like to speak with people who are aware of your criminal record. If possible, we’d like a mix of personal and professional references;
15. If you’re employed and your employer is aware of your criminal record, please include your employer as a reference whom we can contact;
16. A detailed statement stating the reasons for and entering the U.S. – both personal and professional. If you have relatives, their names and State in which they live. Please provide a confirmatory letter from your employer, if your reasons for seeking admission into the U.S. are professional;
17. Documentation concerning pervious waiver applications, whether granted or denied (if applicable);
18. Copy of your resume and any supporting documents such as certificates, diplomas, etc.;
19. Copy of your income tax return for the last 2 years;
20. Most recent T4, in addition to most recent Notice of Assessment and most recent pay stub;
21. Coy of the land title documents pertaining to any property owned by you;
22. Copy of your pardon (if applicable);
23. Pictures of your immediate family; and
24. Any documentation which you think may be helpful to show that you are deserving of a waiver to enter the U.S. such as proof of rehabilitation and good behavior for example:
- Records of participation in rehabilitation programs
- Records of volunteer work
- Records of donations
- Proof of volunteer works
