แบบฟอร์มการแจ้งเรื่องร้องเรียน การทุจริต และประพฤติมิชอบ/Complaint Form for Fraud and Misconduct

คำแนะนำการแจ้งเรื่องร้องเรียนการทุจริตและประพฤติมิชอบ

1. ผู้ร้องเรียนจะต้องใช้ถ้อยคำสุภาพ
2. ผู้ร้องเรียนต้องกรอก ชื่อ-สกุล ที่อยู่ หมายเลขบัตรประจำตัวประชาชน หมายเลขโทรศัพท์ และไปรษณีย์อิเล็กทรอนิกส์ ของผู้ร้องเรียน เพื่อยืนยันตัวตนและใช้ในการติดต่อกลับ
3. ผู้ร้องเรียนต้องกรอก ชื่อหน่วยงาน หรือเจ้าหน้าที่ที่เกี่ยวข้องกับเหตุแห่งการร้องเรียน
4. ผู้ร้องเรียนต้องกรอกการกระทำทั้งหลายที่เป็นเหตุแห่งการร้องเรียน พร้อมทั้งข้อเท็จจริงหรือพฤติการณ์ตามสมควรเกี่ยวกับการกระทำดังกล่าว หรือแจ้งช่องทาง เบาะแสการทุจริตของพนักงานอย่างชัดเจนเพื่อประโยชน์ในการสืบสวน สอบสวน
5. ผู้ร้องเรียนต้องกรอกคำขอของผู้ร้องเรียน
6. ผู้ร้องเรียนต้องระบุพยานเอกสาร พยานวัตถุ หรือพยานบุคคล (ถ้ามี)

ทั้งนี้ เป็นไปตามประกาศมหาวิทยาลัยแม่ฟ้าหลวง เรื่อง มาตรการจัดการเรื่องร้องเรียนการทุจริตและประพฤติมิชอบ พ.ศ. 2563

 

Guidelines for Reporting Fraud and Misconduct

1. The complainant must use polite language.
2. The complainant must provide their First Name - Last Name, Address, Identification Card Number/Passport Number, Telephone Number, and E-mail to verify their identity and for contact purposes.
3. The complainant must provide the name of the division or official involved in the cause of the complaint.
4. The complainant must detail all actions causing the complaint, along with reasonable facts or circumstances regarding such actions, or clearly provide channels and clues of employee fraud for the benefit of the investigation and inquiry.
5. The complainant must fill in their specific request.
6. The complainant must specify any documentary evidence, material evidence, or witnesses (if any).

Note: This is in accordance with the Mae Fah Luang University Announcement on Measures for Managing Complaints of Fraud and Misconduct B.E. 2563.
