City Extension Co-Enrollment: No Credit Repeats payment page

YOU MUST HAVE PERMISSION FROM THE INSTRUCTOR TO CO-ENROLL IN A CLASS.
If you have taken a credit class the maximum allowable times and would like to repeat the class for no credit you can sign up though this on-line form using your Visa, Mastercard or Discover credit card. We do not accept American Express. If you have any questions, please email us: ccsf.extension@ccsf.edu

Only one student and one class per form. If you are registering for another person, you will need to submit a separate form with a different email address for that person.

You must email the instructor for permission before co-enrolling, and cc ccsf.extension@ccsf.edu Once we receive the instructor approval via email, we will email you a code to add to this form. The code is required to process the form.

Questions with a red star * are required, the others are optional.

You will need to provide:
- Your CCSF student ID number
- the course name and number CRN#
- instructor name
- number of units
- semester
Once you have completed payment you will be emailed a receipt. Please retain the receipt to show your instructor.
