Complete this application form to indicate which school your child would like to attend for the 2022-2023 school year.
Families of Current NPS 3rd-5th Grade Students:
If you would like to select an intermediate school for the 2022-2023 school year that is not currently the intermediate school your student attends, please make your selection. You will select a 1st choice, 2nd choice, and 3rd choice.
If you would like your student to remain at the intermediate school they currently attend, please do not complete the form as their enrollment will automatically continue at their current location.
An application must be filled out for each child who will be entering grade 3 next year and any student wishing to change schools in 4th - 6th grades.
Be sure to complete all required fields before submitting. Applications will be processed in the order received based on the date and time of the application submission.
Should more children apply for a school than the school can accommodate, the student's second choice will be selected.
