The Parent(s) hereby agree that the Student noted on this form may participate in after-school activities selected on this form. The Parent(s) and Student acknowledge that some or all of such activities inherently carry risk of potential injury to the Student or loss of Student property. The Parent(s) and Student hereby release the School from and indemnify the School and shall hold the School harmless against all claims, damages, any damage of the property of the Student or injury to the Student, except where such claims, damages, damage to property or injury directly result from the gross negligence of the School.
Por la presente, los Padres aceptan que el Estudiante anotado en este formulario participe en las actividades extraescolares seleccionadas en este formulario. Los Padres y el Estudiante reconocen que algunas o todas estas actividades conllevan un riesgo inherente de posibles lesiones al Estudiante o pérdida de bienes del Estudiante.
