By signing this waiver, I, blanks* , understand my appointment on date of appointment* with the styliststylist name* requires a 48-hour cancellation notice. To hold this appointment I have agreed to pay a deposit of $100 that is refundable up to 48 hours before my appointment start time. If I fail to cancel my appointment before 48 hours, I will forfeit my deposit.
