PHMP is seeking Volunteers to assist in our Mass COVID-19 Vaccination Clinics. If you are able to help in some way, please submit the form below and we will contact you. Thank You in advance!
1. I would be able to assist in the following ways:

I can help with patient assistance (temperature checks, making copies, assisting patients, handing out forms, asking questions), etc)
I am a nurse and can draw up vaccine
I am an EMS worker and can monitor patients
I am a pharmacist or pharmacy intern with a vaccinator license
I can stand outside all day and direct traffic and assist patients
I can assist patients and register them in the online portal
Set up (early 8am to 10am)
Tear down (5pm and later)
