Terms of Service Agreement Template

&#13;&#10;1. Cleaning supplies to perform this service will be supplied by: ___________________________&#13;&#10;2. Total costs of service will be: $_______ and payable monthly on the 1st day each month with net 15 terms.	&#13;&#10;3. This Agreement will expire one year from the date of this agreement, but will automatically renew annually with a 3% inflation price increase if services go uninterrupted.&#13;&#10;4. The party receiving services may cancel the agreement only due to poor service.
