PRIVACY ACT CONSENT/HEALTH & DISABILITY DECLARATION
I/We hereby apply to become a member of the South Waikato Motorcycle Club Inc. and agree to abide by the Constitution and rules of the club. I/We are aware that this membership runs for a 12 month period from 1 April 2026 to 31 March 2027, whereby it is then renewable for the next full membership year. The South Waikato Motorcycle Club Inc. is required to indicate reasons for acquiring this information under the Privacy Act 2020. It is a requirement in our Constitution that the Secretary holds a complete record of all club members under the Incorporated Societies Act, under which the South Waikato Motorcycle Club is registered. This information remains confidential to the Membership Officer & Secretary of the SWMCC. I acknowledge my rights to access and correction of this information. This consent is given in accordance with the Privacy Act 2020.
