1. I understand I am responsible to transport my adopted kitten(s) to the spay/neuter appointment(s) provided by the Rescue. I also understand it may take weeks to receive the appointment after adoption based on availibility of appointments, age and weight at the time of adoption.

*Surgery includes Rabies vaccine and Microchip; as well as Combo Tests when available.*

 ** Cancellation notice is required at least 72 hours prior to the surgery date to avoid a $75 cancellation fee. This fee must be paid prior to rescheduling the missed appointment.**

2. I understand if I choose to take my adopted kitten(s) to my chosen veterinarian I am financially responsible for all costs associated with the surgery. I agree to provide the date of surgery within 30 days of adoption; and understand it must be performed when the kitten is at least 4  months old AND 4 lbs in weight. 

3. I will stay in communication with BTKR to report that this/these kitten(s) have been vaccinated and spayed/neutered when kitten(s) are at least 4 months AND 4 lbs.  I will have any follow-up vaccines or boosters administered in a timely manner as instructed on the paperwork.

4. I will provide BTKR with a copy of the vaccination and spay/neuter record clearly stating the date, description of kitten(s), name of kitten(s), age of kitten(s), and treatments performed within 1 week of the date of the procedure. A picture message of the paperwork delivered electronically is acceptable. The spay/neuter email is sammiebtkr@gmail.com. The US mailing address is 4177 Rainbow Dr., VA Beach, VA 23456.

5. I agree that BTKR has the right, at their discretion, to reclaim this animal if there is reason to believe the vaccinations and spay/neuter have not been done within the above weight requirements unless special arrangements have been made.

6. If I have not completed the vetting at the appropriate time or within a reasonable time after, I WILL return the animal(s) to BTKR per state requirements to have cats spayed/neutered and vaccinated.

7. I give permission for a representative of BTKR to come onto my premises where the animal(s) is kept for the purposes of verifying spay/neuter or retrieving him/her if these conditions have not been met.

8. I agree that this is a legal and binding contract, and by accepting this agreement, I hereby release Billy the Kidden Rescue or its’ representatives, volunteers, or successors from any responsibility for any and all injuries and damages as a result of the adoption of the above described animal(s).

 9. In the event it becomes neccessary to forward this contract to an atgtorner for enforcement, I agree to pay all attorney's fees and court costs incurred by BtKR or its representatives.
