    STUDENT CONTRACT AGREEMENT:

     

    Disclaimer: Students are responsible for researching the specific requirements of their state board. In most states, a cosmetology or esthetics license or enrollment in a state-approved apprenticeship program is required to practice lash extensions. Similarly, a tattoo or permanent makeup permit/license or enrollment in a state-approved apprenticeship program is typically required for microblading. We provide certifications only, not licenses.

     

    While these are common state requirements, students must independently verify their state's regulations by contacting their local Board of Cosmetology and Board of Health. Students must also confirm that they either possess the necessary license or commit to obtaining it prior to offering professional services to clients. CHARMED BEAUTY COMPANY does not issue licenses; this authority rests solely with the state board. If you are currently enrolled in a cosmetology or esthetics program and require credit hours, please confirm with your institution that they will accept CHARMED BEAUTY CO. credit hours before enrolling. CHARMED BEAUTY CO. is not responsible for any issues related to credit hour acceptance or state licensing.

     

    Rules:

    1. The remaining balance of your class payment must be settled via debit or credit card. Cash payments are not accepted. No exceptions.

    2. Professional attire, specifically all black, is required. Please note that photographs may be taken for promotional purposes. Slippers, bonnets, house robes, house coats, blankets, pajamas, or clothing with inappropriate graphic designs are strictly prohibited.

    3. Should you be unable to provide a model, you must notify us. Certification cannot be granted without a model. For any inquiries, please text or call 706-372-0277.

    4. Upon submission of this form, a receipt will be sent to your email address, serving as your confirmation.

    5. Please arrive with a positive attitude and high energy.

    ***A model is mandatory for the Microblading Course. No exceptions.***

    Approximately 2-3 days prior to your scheduled class, you will receive an email and/or text message detailing the location and times. We appreciate your patience. If you have not received communication from us within 24 hours of your class, please call 706-504-1146. Thank you.

    MICROBLADING Students ONLY: 

    You MUST have a model to receive your certification for Microblading.

    Please ask your model these questions to see if they qualify to be your model 

     

    •  Pregnant or Nursing

    •  Those prone to keloids or post-inflammatory hypopigmentation

    •  Diabetes (will need medical clearance)

    •  History of MRSA in the last 6 months

    •  If you have a transmittable blood disease such as HIV or Hepatitis

    •  Abnormal Heart Condition (will need medical clearance)

    •  Patient's undergoing Chemotherapy/Radiation 

    •  If you have any kind of skin condition on or near your eyebrows. This includes eczema, shingles, rashes, or anything else near eyebrows.

    •  Very Oily Skin

    •  Taking Accutane (has to be off Acutane for at least a year)

    •  Taking Blood Thinners such as: Aspirin, Ibuprofen, Coumadin etc.

    •  Allergic reaction to alcohol or numbing agents

     •  Those with dark pre-existing eyebrow tattoos or permanent makeup (please let the Instructor know)

    •  Cannot be under the age of 18 years old

      Artist Shadow Day for Permanent Brow tattooing   was created to help students with additional educational support. You will able to watch and ask questions on the service in which course you have previously taken with Cami. You will have the option to purchase a training kit. Please note. This is not a full training class. This is only a look-and-learn with the option to ask questions for guidance and support. The total fornthe Learning day is $300. We will provide a small mapping kit for you to use you are responsible of finding your own model to bring to the class. We are not responsible for finding a model for you. This class we will go over detail how to properly map brows. We will hit all topics of brow shaping from beginner to Advanced levels. This class is set for 2 hours.
