Model Requirements for 𝗠𝗶𝗰𝗿𝗼𝗯𝗹𝗮𝗱𝗶𝗻𝗴/Brow HENNA 𝗖𝗼𝘂𝗿𝘀𝗲𝘀 𝗢𝗡𝗟𝗬 (*Lash STUDENTS will NOT need a live model) * This field is required.Do you understand that you will need to bring a model 18 years old or older for Brow Henna classes and on the 2nd day of training for Microblading/Microshading students. To receive your Certificate after class. We are NOT RESPONSIBLE for finding a model for you.

All STUDENT CONTRACT AGREEMENT:

Disclaimer: students MUST do their due diligence to research what the state board requirements are for their specific state. In most states, you must have a cosmetology or esthetics license or be in a state-approved apprenticeship program to practice lash extensions and have a tattoo or permanent makeup permit/license or be in a state-approved apprenticeship program to practice microblading. I only provide certificatiins. Not licenses.

Although these are what most states require, STUDENTS MUST RESEARCH THEIR STATE'S REGULATIONS on their own by checking with the Board of Cosmetology and Board of Health in their area. Students must also agree that they either already have their license, or agree to obtain it before they begin servicing clients professionally. CHARMED BEAUTY COMPANY does will NOT license students(only the state board can license). If you are in a cosmetology or esthetics program and in need of credit hours, check with your current school first before enrolling to be ENSURE they will accept the CHARMED BEAUTY CO. credit hours. CHARMED BEAUTY CO. is not responsible for any students not being given credit HOURS OR LICENSING THROUGH THEIR STATE.

Rules:

1. Remainder of your class payment MUST be paid with debit or credit card. No CASH. NO EXCEPTION.

2. You MUST wear all black (photos may be taken for promotional content). Absolutely no slippers, bonnets, house robes, house coats, blankets, pajamas or inappropriate graphic designs or clothing.

3. You must contact us if you CANNOT to bring a model. YOU CANNOT BE CERTIFIED WITHOUT A MODEL. Any questions please text or call 706-372-0277

4. Upon submission of this form an email to receipt will be sent to you that will be your confirmation.

MICROBLADING Students ONLY:

You MUST have a model to receive your certification for Microblading.

Please ask your model these questions to see if they qualify to be your model

• Pregnant or Nursing

• Those prone to keloids or post-inflammatory hypopigmentation

• Diabetes (will need medical clearance)

• History of MRSA in the last 6 months

• If you have a transmittable blood disease such as HIV or Hepatitis

• Abnormal Heart Condition (will need medical clearance)

• Patient's undergoing Chemotherapy/Radiation

• If you have any kind of skin condition on or near your eyebrows. This includes eczema, shingles, rashes, or anything else near eyebrows.

• Very Oily Skin

• Taking Accutane (has to be off Acutane for at least a year)

• Taking Blood Thinners such as: Aspirin, Ibuprofen, Coumadin etc.

• Allergic reaction to alcohol or numbing agents

• Those with dark pre-existing eyebrow tattoos or permanent makeup (please let the Instructor know)

• Cannot be under the age of 18 years old
