Thanks for your interest in getting tattooed at Moo Tattoo!
This form is to request an appointment. The appointment is finalized after we confirm and a deposit is received.

We ask that the person(s) getting tattooed to be the one who makes the appointment request. This ensures proper communication between us and the client.

You must be 18 years of age or older with a valid state or federal ID. Parents can not "sign" for a minor.

When selecting the artist:
"First Available"-First available artist to provide the fastest response.
Noah- 7 days a week upon availability
Bash- Friday-Monday
Staci- Friday-Tuesday

A Deposit is Required-A link will be sent to make a $40 non-refundable deposit for the tattoo appointment. This deposit reserves your spot for the time and date agreed upon, and is deducted from the price of your tattoo.

IF YOU DON'T RECEIVE AN EMAIL, PLEASE CHECK YOUR SPAM FOLDER. Due to the form entry, sometimes the response gets sent to 'junk or spam mail'.
