This form is to request to use entitlements you have accrued from your Restaurant Manager, not to ask for time off. Leave Requests and NAs are managed through MyJob.
Requests must be in prior to 7am on the Monday that Payroll is processed for the week that has just finished. Week Endind date refers to the date of the Sunday in the week you wished to be paid for.
Please note that entitlements can not be paid to you without correct completion of this form. This form must also be utlised when you wish to take time off without pay.
