  • WELCOME to Proper Prep Preschool's official registration form!  I am so excited you have chosen me, Ms. Cam, to be your child's preschool teacher.  

    Below are a few key notes I would like for you to be aware of in order for you and your child to be as prepared as possible for school.

    Basic Information

    • Children's must be 3 years-old 
      -OR must turn 3 by October 1, 2022
    • Ms. Cam strongly encourages children to be potty-trained by the first day of school.
      -If your child is not completely potty-trained, you will need to provide diapers, wipes, etc. for your child.
    • Children must arrive with a bag (ex: small book-bag) with the following items:
      -Water bottle/juice cup
      -Lunch
      -Extra change of clothes
      -2-3 extra pairs of underwear
      -Any other items you feel your child may need

     

    School Year Information

    • Open House - Tuesday September 6, 2022 from 5:00-5:45pm
    • 1st DAY OF SCHOOL - Wednesday Septmember 7, 2022 
    • Upon registration you will receive an outline of the school year, which will include: day-to-day schedule, learning units that will be covered through out the school year, important dates to be noted, etc.

  • COVID-19 Liability Waiver

    I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
    I further acknowledge that Proper Prep has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
    I further acknowledge that Proper Prep can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, salon staff, and other salon clients and their families.
    I voluntarily seek services provided by Proper Prep and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
    I attest that:
    * I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
    * I have not traveled internationally within the last 14 days.
    * I have not traveled to a highly impacted area within the United States of America in the last 14 days.
    * I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
    * I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
    * I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
    I hereby release and agree to hold Proper Prep harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Proper Prep. I understand that this release discharges Proper Prep from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Proper Prep. This liability waiver and release extends to the salon together with all owners, partners, and employees.

  • Illness Rules & Requirements

    1. Should your child have a temperature over 99.9, Proper Prep will contact you and require that you remove your child from school for the day.

    2. Should you, your child, your child's siblings, or any other individuals living in your home present any signs of illness, Proper Prep has the right to decline your child's attendance for at least 72 hours after all person(s) are symptom free.  Illnesses include, but are not limited to the following:

    • Vomitting
    • Diarhea
    • Fever exceeding 99.9
    • Being treated by an antibiotic for less than 24 hours 
    • Etc.
  • Informed Consent and Acknowledgement

    I hereby give my approval for my child’s participation in any and all activities prepared by Proper Prep during the selected camp. In exchange for the acceptance of said child’s candidacy by Proper Prep, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Proper Prep and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected school days.

    In case of injury to said child, I hereby waive all claims against Proper Prep, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

    BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

  • Payment Information & Tuition Option

    Tuition is due on the 5th of every month, beginning on October 5, 2022 and ending on May 5, 2023.  Each month, a tuition invoice will be emailed to you.

    In order to complete the registration process & reserve your child's spot with Proper Prep, you must pay first month's tuition.  This payment will count as September tuition, which is why the tuition invoice schedule does not begin until October 5th.
