You previously retained the Law Firm of Kuczek & Associates (collectively known as the "law firm") to perform estate planning services for you.  Those services include a review of your assets and liabilities, meeting with you to determine your estate planning objectives, and the preparation of various documents, including wills, trusts which will affect your relative rights to each other's property during life, disability and at death.

We understand now that you have, or are in the process of separating / divorcing.  Going forward, we believe it is permissable for our law firm to represent either of you, or even both of you, if you chose to update your estate planning documents with us i.e. we don’t believe there is a conflict of interest.  You will be treated as two separate clients and your information would be completely confidential from each other. 

If that is acceptable to you, we ask each of you to please sign below and submit this form to us.  Then contact us at your convenience to discussing updating your documents. 

If at any time, we feel that a conflict of interest arises in our representation, we will inform each of you and recuse ourselves, at which time each of you will need to find a different attorney.
